The NRC was notified of a potentially reportable deficiency in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.

j Due to the extensive investigation and evaluation required, an Interim Report is attached.

It is now expected that this inf ormation will be finalized by November 2,1984, at which time a complete report will be submitted.

Potential-Problem-During _a Quality Assuranee Audit ' of WPP/QCI 207.0, Rev. 9,

" Disassembly and Reassembly of ' Quality Class 'Q','. 'R', and 'S '

Valves," a number of. valves were discovered with bolting deficiencies. Audit #QAF 101-S-84-37-A' reviewed f orty-one valves. The review discovered loose and/or missing hex: bonnet nuts in twelve valves and diaphragm valve stud nuts which were in a less than flush condition in the valve ~ body and/or nut in nine. valves. The ~ audit also reported that specification 13-PM-204 f or " Nuclear Field. Fabrication and Installation of Piping Systems" and Procedure.WPP/QCI 207.0- for ;" Disassembly and Reassembly of Quality Class

'Q',

'R', and,'S' Valves" lack the-requirements of the ASME code to define the proper thread engagement.

II.

Approach To and Status Of Proposed Resolution

1. The deficiencies of the valves found during the QA audit will be reviewed with the respective. vendors to determine the adequacy.of the valves in the present condition.
2. Bechtel Construction is to develop a reinspection program E

to determine that this is 'n isolated situation._

3. The. specification and WPP/QCI will be reviewed along.with the ASME code to determine if more clarification is-required.

III.

Projected Completion of Corrective Action and Submittal of the Final Report Bechtel Construction is currently evaluating QAF 101-84-37-A.

One. portion of the' evaluation included reinspecting,a percentage of valves in Units 2 and 3 to determine that:the.

cases listed above are isolated cases.. This reinspection was -

initiated under WPP/QCIW569.0 and 570.0 The results of the-reinspection program are now being evaluated. Construction is also evaluating.the valves which were discovered with bolting deficiencies. Engineering-is reviewing the specifications to determine if clarification is required. The evaluation for this review and the final report are forecast to be completed by November 2,.1984
