Calvert Cliffs Nuclear Power Plant Unit Nos.1 & 2; Docket Nos. 50-317 & 50-318 Annual Report of Power-Operated Relici and Safety-Valve Failures and Challenges for the January 1 through December 31.1991 Period Gentlemen:
We are submitting this report in accordance with Calvert Cliffs Technical Specification 6.9.1.5.c. Tbc report descriacs challenges to, and failures of, Power-Operated Relief (PORV) and Safety Valves.
Two PORVs actuated in 1991.
The first actuation occurred on June 5,1991. Uni: 1 PORV 1-ERV-404 lifted while at COLD SHUTDOWN (MODE 5) with the reactor coolant system depressurized and vented. This occurred while a technician was installing a control-circuit jumper as part of a Surmillance Test Procedure (STP). Ile accidentally shorted the jumper to a power supply terminal during installation. This caused the power-supply fuse to fail and the PORV to actuate.
On June 21,1991, while performing an STP at MODE 5, Unit 1 PORV 1-ERV-402 cycled rapidly several times. This occurred when a jumper, which had a stray wire strand, was shorting to ground.
The PORV leaked by its seat as a result of this rapid cycling. The PORV was intentionally cycled once, shortly aftc : test completion to stop the leakage.
These events were investigated as part of our correctise action program. Appropriate corrective action was taken in both instances.
