Occurrence Date:
April 1.1, 1974 L
Facility:
Peach Botton Atomic Power Station R. D.1 Lolta Ibnnsylvania 17314 Identifict. ion of Occurrence:
Inoperability of PS-2-5-14C and PS-2 + 14D Conditions Prior to Occurrence:
Plant at 2% Power Description of Occurrence:
During the pinnt start-; a to greater than 30% power, the operability of the preccare ewitchen which cence turbine first stage pressare were tonitored.
It was rated that all the prermure evitches failed to operate at the 30%
power level. Before proceeding up in power above 30%, the setpointo on the e
cwitchee were adjunted to provide the proper logic input to the RPS aystem.
During the subsequent chutdown, these pressure cwitches were recalibrated
[.
in accordance with a new curve of first stage turbine pressure vercus power based on data obtained during the previous operation. Duving the next start-up, the C and D pressure evitches failed to operate at the correct turbine power level and were again readjunted to provide proper logic input S$
before exceeding 30% power.
Subsequent investigation determined that the s
@7 sensins line from the turbine was not delivering the proper preocure signal 1
J
$8 to th-ce two evitches. Because. plant conditions prevented access to portions n
bg of the sensing line, the wires to the proper ' logic relays were lifted to'~

provide the proper EPS logic inputs.
4 Designation of Apparent Cause of Occurrence-1 3@
It is presently believed that a blockage exists in the semaing line frem d

There are to cafety implications aceociated with this occurrence because the

Corrective Action:
On each occassion.vb-proper relay operation did no cur, setpoint '
adjusteents were ca
. r 1cado were lifted to provid-4 roper inputo to the RPS logic. These action; woro conservative in that they ensure a reactor ceras fron turbine trip at any power level (not only above l
30% power). Final corrective action cuot 'avait the next plant shutdown.
