An defined by Technical Specifications for the Vermont Yankee nuclear Pouer Str.tlon, Sectica 6.7.B.1, t:c are reporting the follouing Abnorm:tl Occurrence as A0-74-06 nnd conflrning our telephone, conversation with your office.
At approninatoly 1115 on April 18,'1974, while caintenanco van being perfort:ed on the Reactor Euilding Exhnust Fans, hotb recess doors to the north entrcace of the Reactor Building were discovered to have been Icft partially open.
This situation existed for approxirately two minutes prior to both doors befug closed.
It ucs datermined that this conditicn was In violation of Techniccl Specifications, Section 3.7.C in that the integrity of the' secondary containrent system was not maintained during that period of tiro.
The reactor van operating at 83.47. power at the tire of this incident.

An initial review of circumstances Icading to the incident indicated that the access doars uere not verifica cloc.cd by plant personael utiliaing them during this period of Lima.
Subsequent action uill be to investigate a means of core positively asauring ful.1 door closus e.
