This Special Report is being submitte,d pursuant to Millstone Unit 3 Technical Specification 3.3.3.3.a, Seismic Monitoring Instrumentation. This Specification requires a Special Report to the NRC within 10 days following one or more of the seismic monitoring i

instruments shown in Table 3.3-7 being inoperable for more than 30 days. This report outlines the cause and the plans for restoring the instrument to operable status.

The plant shut down for a refueling outage April 14,1995. On April 16,1995, at 2050, with l

the plant in Mode 5 (Cold Shutdown) at 0% power, and 185 degrees Fahrenheit, the Seismic Monitoring System (SEI) was declared inoperable for an Automated Work Order (AWO) action. The work Involved a plant modification to relocate a seismic triaxial peak accelerograph to a different location on the Containment Safety injection Accumulator Tank. The Technical Specification Limiting Condition for Operation 3.3.3.3.a was entered on April 16,1995. However, no work was actually done to make the seismic instrument inoperable until May 16,1995. During this 30-day interval, the instrument remained Installed and capable of performing its function, although declared administratively inoperable.

On May 16,1995, the seismic monitoring system was taken out of service for instrument calibrations. The total period of time the detector was declared inoperabMor a work order, followed by the monitoring system being out for calibration, exceedeo V) days.

Prior to start-up from the current refueling outage, the detector will be restored to operable status in its new location.
