d-I SPECIAL REPORT 91-03 LOOSE-PART DETECTION SYSTEM CHANNEL l@PJRABLE FOR CR'MTER THAN 30 DAYS This report is submitted in accorr'ance with Technical Specification 3.3.3.8 Action (a) - Sich states. "k'ith one or more Loose Part Detection System channel (s) inoperable for more than 30 days, prepare and submit a Special Report to the Commission pursuant to Specification 6.9.2 within the next 10 days outlining the cause of the malfunction and the plans for restoring the channel (s) to OPERABLE status" At 0900 CDT on 8/20/91. following receipt of a Main Control Board Loose Part Detection System annunciator and performance of an intervice inspection by Instrument and Controls technicians and licensed operators, Channel 1 of the Loose Part Detection System was determined to be inoperabic. The plant was in Mode 1 at 100% power.

The Channel 1 accelerometer is located in the containment mounted on the lower part of the r ector vessel near the incore instrunentation. Initial troubleshooting of Chaoel I has determined that the problem lies with the sensor mounting or assoct ted sensor cabling. Since the instrument. sensor is connected to the reactot vessel head (thus inaccessib!c during power operations), further troubles ;oting vill not be performed until the next outage of sufficient duration. This is the second channel that has become inoperable; Channel 3 became inoperable on 6/20/91 (reference Special Report 91-02). The remaining ten channels in this system remain operable.
