This condition was reported via telephone to Mr. J.-Wiggins of your staff on. auary 20, 1983 and was followed by interim ~ reports dated February 23, 1983 and August 9, 1983.

Description of the Deficiency i

A review by-Niagara Mohawk Quality Assurance identified that required notes concerning separation criteria, which appear on the system elementary diagrams, were-not included on.the. corresponding-connection diagrams or implemented in the connection diagrams wiring.

Therefore, these requirements.

werejnot incorporated into the PGCC hardware.

Analysis of Safety Implications i

l This deficiency resulted from a General Electric Quality Control and Engineering failure-to identiff all separation _ criteria deficiencies prior.to shipment to the Nine Mile Point Unit 2 site.

There.is a possibility that if this problem.were to have remained' uncorrected, it could have adversely affected the. safe-' operation of the plant.. 'A detailed analysis _of Safety Implications was not performed.

Corrective Action General-Electric has added separation. notes'to several connection diagrams asL a result of this deficiency. Subsequent to this action, Niagara Mohawk j

Quality Assurance performed a review of all' safety related connection-diagrams requiring electrical separation and found the required separation notes.

i General Electric has corrected all the monitoring and tes't relay; wiring located at the General Electric' shop except for one relay.

The remaining

  • relay will-be corrected at the Nine Mile l Point Unit 2 site by_ June 30, 1984;.
