O'Reilly Special Report Page Two July 25, 1983 ACTION TAKEN At 2100 hours0.0243 days
0.583 hours
0.00347 weeks
7.9905e-4 months
on July 12, 1983, with the Plant in Mode 1, Main Steam Line Radiation Monitor RMG-19B was declared inoperable because of spurious alarms with fluctuating meter indications.

RMG-19B is required to be operable in Modes 1 through 4.

Therefore, Action Statement 30 of Technical Specification 3.3.3.1 applied.

The preplanned alternate method of monitoring was implemented on July 13, 1983, at 2100 hours0.0243 days
0.583 hours
0.00347 weeks
7.9905e-4 months
, in accordance with Action Statement 30.

Personnel investigation of the spurious indications on the radiation monitor determined that the low range circuit board was causing the signal fluctuations.

CAUSE OF OCCURRENCE The cause of the occurrence is attributed t'o a component failure on the low range circuit board of RMG-19B.

PLANS AND SCHEDULE FOR RESTORING TO OPERABLE STATUS The projected date of repair of RMG-19B is dependent on the receipt of replacement parts which are presently on order from the manufacturer.
