Asas z-Detroit Edison DRIOD FERMI UNIT 2 PROIECT QUALITY ASSURANCE De mber 9, 1982 WISMER AND BECKER'S QA PROGRAM As of the first quarter of 1982, Wismr and Becker has undergone substantial hanger program changes. These program changes consis 3 of procedural as ml1 as personnel attitude changes.
The procedural changes resulted in clarification of inspection requirements as well as adding inspection requirements in order to docunent the adequacy of tha installation.

Wisner arxl Becker's procedure WB-C-105 "Concmte Anchor Installation and Testing", Rev. 20, was issued on February 23, 1982 to mquire an inspection and docunentation of concrete anchor spacing and abandoned hole requirenents before installation of the base plate.
(See WB-C-105, Para. 3.2) 2.
The Wisner and Becker's procedure WB-O-122 "QA Docunentation Review" was changed via ICP #482 to include a hanger documentation review. This change includes an actual checklist to be used during the review of hanger documentation packages.

Wismer and Becker's procedure WB-C-114 was revised to include skewed fillet weld inspection criteria. This was approved April 15, 1982.

Wisner and BEcker is currently in the process of revising their Hanger Inspection procedure to provide acceptance criteria for inspection of strut and/or catalogue ccrnponents.
The improved personnel attitude changes are indirectly a result of the above procedural revisions as well as directly attributed to the NRC's Construction Assessment Teams survey, June 21 through July 2, 1982. As a result of this survey, Wismer and Becker's Management has initiated a number of Corrective Action measurenents. These corrective action neasurenents, illustrated in Wisner and Becker's response to th3 licensee regarding the CAT Team audit, have had a direct impact on the contractor's Inspection Program (See copies enclosed).

e To provide a reliable basis for deciding whether or not an additional program should be integrated into ongoing programs, and to assure that all remaining potential areas of concern are addressed, a sampling program providing a statistically sound basis for decisions was pro-posed to management.
The program was basically endorsed and will be implemented as soon as plans are finalized (December, 1982). The result of this inspection program will determine whether or not additional inspections should be made on hangers installed prior to March, 1982, and the character-istics to be inspected.

Summary of Programs Planned A.
Continued surveillance of contractor's activities to assure programs effectiveness for new installations or modifications.
B.
Implementation of sampling programs by extending contractor's inspections of work done on "old supports" to include all characteristics of supportt. This program will continue until statistical significance is achieved (4-275 hangers).
J The results of this effort will be analyzed by PQA and will be the basis for concluding whether or not additional support characteristics need to be included in ongoing programs, and thus substantiate or disprove effectiveness of Contractors' inspection programs in the pre-March, 1982 period.

This refers to the discussions between Mr. W. E. Holland and others of the Detroit F4ison Company staff and Messrs. E. B. Little and I. T. Ein of this office during a meeting at the Femi site on December 10, 1982.
These discussions related to deficiencies your site QA staff identified during a review of large bore ha"Eers previously installed and accepted by site contractors quality control. With regard to the natters discussed, we understand that:

Large and small bore supports installed and QC accepted prior to December 9,1982, vill be inspected and all deficigncies vill be evaluated by an independent professional engineering organization under control of the licensee.

The activity described in Item 1 vill not be initiated until the program and schedules are presented to ERC Region III.

All future support installations vill be reinspected 200% by PQA based or selected inspection cht.rseteristics for burdw%re and softvare subsequent to the Contractor QO acceptance. Eev support installations issued to all construction crews vill be limited to 100 packages with no more than 40 large bore piping supports.
