-N HIGHLIGHTS OF WEEKLY INFORMATION REPORT-WEEK ENDING JUNE 15, 1990 Comanche Peak Steam Electric Station. Unit'1 On June 6, 1990 TV Electric concluded its self-assessment operational period at the 50% test plateau, following completion of.a successful remote shutdown test. A nine-person inspection team composed of staff from Region IV, other regions, NRR, and contractors and led by J. Weibe, has been examining TU Electric's readiness to operate Comanche Peak, Unit 1 above 50% power. The inspection t m has been evaluating, among other things, TV Electric's self-assessment at the 50% power plateau, which began on May 29 and was scheduled to last for at least one week. The team also inspected the closeout of previous ORAT items, operations, maintenance and surveillance, startup test activities, and the safety assessment-quality verification corrective action stogram.
Although the inspection team's onsite inspection activities have seen concluded.

Nucleac Safety Research Review Comittee Meeting On June ~4-5, 1990, the Nuclear Safety Research Review Comittee (NSRRC) held a Full Comittee meeting in Rockville, Maryland. RES' responses to the Committee's third and fourth reports, a review of ongoing RES 1rograms and budget. planning actions were the focus of the meeting during t1e first i
day.
The second day of the meeting opened with a changing of the guard.
Dr. David Morrison was elected new NSRRC Chairman to replace the first NSRRC Chairman, Dr. Neil Todreas,-whose outstanding leadership and high standards for excellence caused the Committee to be extremely productive during its first years of operation. The meeting concluded with discussions of how NSRRC will respond to the EDO request regarding advice to RES on the size and content of the nuclear safety research program and Comittee plans t
for the coming year. The final NSRRC meeting for FY 1990 has been scheduled for September 25 and 26, 1990, in Bethesda, Maryland.
~NRC Staff Observation of Surveillance of DOE Yucca Mountain Project Office During the period June 6-8, 1990, the Division of High-Level Waste Management staff observed a DOE Yucca Mountain Project Office (YMPO) internal quality assurance surveillance of YMP Study Plan review activities. The NRC team consisted of QA staff who evaluated both the YMP QA program in the area of Study Plan reviews and the effectiveness of the audit process.
The DOE /YMPO surveillance team was adequately prepared and performed a thorough l
review which effectively identified some minor deficiencies in the YMP Study i
Plan review program. Although the reviews of Study Plans required by procedures had been performed,-a fairly large number of minor, procedural problems were identified by the surveillance team.

' Items of Interest Week Ending June 15, 1990 Brunswick Units 1 & 2 Requalification Issue - Update On June 10, 1990, the Regional Administrator granted Arolina Power and ight (CP&L) C: ;,sny ap)roval to restart the Brunswick Units. The approvai me after two cut of t1ree crew., successfully passed NRC administered simulator linations which were given on June 9 and 10. After having demonstrated to J
/RII staff that both units could be safely operated with four shifts (now w osed of the two crews which recently passed and two others which passed NRC
.inistered examinations in May) Unit 2 was restarted on June.11 and Unit 1 restarted or June 12.
CP&L will inform the NRR/RII staff prior to June 15 of their plan to qualify a fifth operating shift crew.
In regard to the examination failures which prompted CP&L to shut down both
-units on May 20, 1990, CP&L is r;esently conducting a root cause aralysis. The results of this root cause an: lysis and the associated long-term corrective action plan,.which will be incorporated into the Brunswick Integrated Action Plan, will be submitted,for staff review by June 30, 1990.
A regional PN was issued on the restart.

.On June 9. 1990, the un t was holding at about 50% power in the process of i
restart from the 12th refueling / maintenance outage. A series of problems were encountered with the offgas hydrogen recombiner. -At about 1400, the "B" recombiner malfunctioned and-the hydrogen analyzer pegged upscale (75%). The offgass flow increased from 30 to 225 scfm.
During the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />, the. licensee attempted to get the recombiner operating properly, but was unable to keep the recombiner in operation for more than about two hours at a. time. At 0238 on June 10, 1990, plant management directed the operators to reduce power 250 MWe (37%). By 0515, there were indication of improving conditions, with the offgass flow at 85 scfm and a hydrogen concentration of 3.52%. However, at 0725 on June 10, the offgas flow rate, the hydrogen analyzer, and the charcoal vault temperature all went to full scale. Shortly thereafter, a plant equipment operator, making outside rounds, notified the control room that he had felt vibration and had heard loud noises coming from the offgas building.
The operator began reducing power to stop the offgas flow and, at 0826 on June 10, inserted a manual scram. A red phone notification of the scram was logged at 0920.
A nitrogen purge of the system was started at 1025.
Since post-treatment readings of C0 and CO, indicated the possibility of a fire in the charcoal beds, the licensee declared an unusual event.
San Onofre Nuclear Generating Station,
' Southern California Edison Company is continuing its weld-o-let installation for both San Onofre Unit 3 steam generator feedwater distribution boxes.
Additionally, a decision has been made'to modify the feedwater ring piping supports inside the steam generators.
The current outage schedule for San Onofre Unit 3 has now been delayed ten additional days, indicating breaker closure on the grid on July 11 rather than-July 1.
This means that the expected shutdown of San Onofre Unit 2 will occur about July 17.
It is expected that Unit 2 will be shutdown for approximately 34 days'to inspect and repair.
Finally, San Onofre Unit 2 surpassed San Onofre Unit 1 in cumulative gross generation last week. As of June 12, 1990, Unit 2 had 48.02 billion KWH, while Unit 1 had 47.89 billion KWH.
Unit 3 currently is at 43.54 billion KWH.
Seabrook Unit 1 Edward A. Brown a'nounced on June 8, 1990, that he will retire on October 1,
.1990, from his position as President ohd Chief Executive Officer of New Hampshire Yankee.

at Seabrook Station upon his retirement. Mr. Feigenbaum joined Seabrook Station in 1984.
ASME' Inservice Testing Requirements On the June 6, 1990, the ASME Board of Nuclear Codes and Standards (BNCS) had a meeting and on their agenda made a decision regarding the jurisdiction for
. inservice testing requirements of pumps, valves, and snubbers. These require-ments have been under the jurisdiction of the ASME Code,Section XI, since their incorporation into the Code in the mid-1970s.
Since that time Section XI activity in these areas have been low and the requirements have been virtually unchanged.
The ASME Operations and Maintenance (0&M) organization was formed in the mid-1970s.and has developed new standards for testing of pumps, valves, and snubbers. Their incorporation into Section XI, required approval of the standard (s) by the entire O&M organization and the Section XI organization.
-This process was found to be unworkable because of the time required.
To resolve this problem, the BNCS on June 6 approved a motion to assign the complete jurisdiction to O&M for writing, maintaining, and interpreting inservice testing requirements. The expected near-term result is a new 0&M Code on inservice testing (IST) requirements for pumps, valves, and snubbers.
The.NRC will, in the future, have to incorporate this new O&M Code by reference

in 10 CFR 50.55a to adopt the latest IST standards. This action will be necessary since Section XI will at some point drop the IST requirements which it presently contains.
STS Development
.May 1990 marked the achievement of a major milestone in developing the new STS.
The staff completed the processing of approximately 3,900 comments and a series i
of approximately 65 noticed meetings with the Owners Groups to discuas the results'of the staff's review of the new STS. The discussions addressed all Limits, Applicability, Limiting Conditions for Operation (pplication, Safety licensing portior.s of Technical Specifications: Use and A LCO), Surveillance
' Requirements (SR), Design Features, and Administrative Controls.
The major. remaining portion of the new STS development includes the Owners j
Groups' revision and re-submittal and the staff's review of the Bases.

4 Office for Analysis and Evaluation of Operational Data Items of Interest Week Ending June 15, 1990 Division of Operational Assessment (DOA)
On June 15. the AE0D Director and IRB staff gave a tour and briefing on the
-Operations Center to senior members of-the Omaha Public Power District.
During the period June 11-15, the Zion Diagnostic Evaluation Team (DET) completed' two weeks of-onsite evaluation. On June 15, Al Gibson, the DET Manager, briefed' station management representatives on the Team's prelininary technical and programmatic observatioas developed from the team's firat two weeks of onsite evaluations. The DET will return to Headquarters on Juni 18 for a week of in-office evaluation end planning for the third week back to the site scheduled for June 25-29.
~ 0n~ June 5-8, the second ineetug of JCCCNRS Working Group 9 met at the TTC-in Chattanooga,'Tennessea. This working group is associated with diagnostics, analysis equipment, 2nd systems for supporting nuclear plant operators. The working group last met in the Soviet Union in December 1989. The US Co-Leader for JCCCNRS Working Group 9 is Kenneth A. Raglin, Director, TTC. The Soviet Co-Leader for JCCCNRS Working Group 9 is Victor M. Dmitriev, Department Head, All-Union Research Institute for Nuclear Power Plant Operations (VNIIAES).
Presentations were given on the organization and staffing of nuclear power plants in the US and USSR,_ training and certification of Soviet nuclear plant staff, Soviet diagnostic systems, technical components of US emergency operating procedures, US experience in Safety Parameter Dispicy Systems (SPDS),
-a typical emergency operating procedure training scenario, procedure violations at US nuclear plants, US lessons learned from emergency operating procedure
-(EOP) inspections, and an expert monitor system.
The NRC E0P simulator training.
was observed by the Soviet working group members. The SPDS displays were observed by and explained to working group participants on the General Electric, Westinghouse, and Babcock & Wilcox simulators located at the TTC.
Industry expert system demonstrations, arranged through the Electric Power Research Institute, were provided on the Emergency Operating Procedures Tracking System (EOPTS) and the Reactor Emergency Action Level !!onitor (REALM) System. A Soviet-working group member also demonstrated a decisionmaking expert system support for diagnosis and localization of equipment failures at Soviet nuclear power plants. A short tour of the Sequoyah nuclear plant, located near Chattanooga,
.was also arranged for the working group.
Division of Safety Programs (DSP)
A preliminary case study report " Solenoid Valve Problems at U.S. Light Water Reactors" was issued for peer review. The report presents information on more than 25 events in which common-mode failures or degradations of over 600 solenoid-operated valves were affected, or had the potential to affect multiple safety systems or multiple trains of individual safety systems.

Michael McCann, Region III, Earl Wright,-Region 11, and Ronald Wascom, Louisiana, conducted an introduction to licensing practices and procedures course on June 11-15, 1990 in Atlanta, Georgia. The course covered a variety of licensing itams including

Kathleen Schneider, State Programs, and staff from Low-Level Waste Management assisted in the review.
Freliminary staff findings will be reviewed by Regional and headquarters offices and the results of the review will be con-firmed in a letter to the State.
National Energy Strategy Economic Policy Council Work Group Meeting Harold Denton, Director, Office of Governmental and Public Affairs, and Spiros Droggitis, State Programs, attended an Economic Policy Council Working Group meeti.ng on the development of the National Energy Stretegy (NES) on June 13 1990 at.Departnent of Energy Headquarters in Washington, DC. The purpose of the meeting, which was chaired by DOE Secretary Watkins, was to approve a plan to complete analysis of (ptions for the NES, including the organization of interagency working groups to conduct the-analysis.
. Meeting with Ohio Officials Roland Lickus, Director, Region III State and Government Affairs, met with a subconriittee of the Utility Radiological Safety Board ot Ohio on June 13, 1990 in Columbus, Ohio. The purpose of the meeting was to discuss potential memoranda of understanding with the NRC.

Minister Chung indicated that he is looking for ways and means to significantly increase Korea's science and technology cooperation, including possible expansion of Korea's safety research activities with the NRC.
He promised to convey the results of the Korean investigation into the Amersham incident to NRC as soon as possible.
On Friday, Mr. Lars Hoegberg, Director General of the Swedish Nuclear Power

Visit to URENCO Plants Raymond Brady. Director, Division of Security, and representatives of HMSS, visited URENCO's uranium enrichment plants located at Almelo in the Netherlands-(NL), Gronau in the Federal Republic of Germany (FRG) and at Capenhurst in the-United Kingdom (UK) June 4-8, 1990. fir. Brady was briefed on the plants' security-classificathn programs and discussed matters of mutual interest with NL, FRG, UK, Louisiana Energy Services (LES), and URENC0, Inc. personnel. The visit and related discus: ions will materially assist SEC in providing overall security-classification ad d e and assistance regarding the LES project in Homer, Louisiana.
Visit to Vienna, Austria During the week of June 11, 1990, Lois Telford, SEC, travelled to Vienna, Austria to participate in' negotiating the Phase III agreement and upcoming meetings between the U.S. and the three URENCO Governments (UK, FRG and Netherlands). Other NRC representatives in attendance were Royal J. Voegele.

In addition to negotiating the agreement, the
-agenda _and time frame for the July meeting at OWFN of classification experts from the URENCO Governments were discussed. The week of July 23rd was acceptable to all parties. Meeting dates during that week and a list of attendees will be confirmed with the U.S. by the Dutch Government.

NUDOCS/LPDR Microfiche Program IRM'has completed its review of a Statistica cost estimate and technical proposal to produce additional sets of NUDOCS microfiche for the 76 full-service power reactor Local Public Document Rooms (LPDRs).- It is antici-pated that the project will meet its scheduled start date of-July 1, 1990.

Decommissioning Records The Office of Information Resources Management staff is working with NMSS in reviewing terminated Test ar,d Research Reactor case files to determine the completeness of the documentation on decommissioning activities.
IRM will separate.the decommissioning records contained within about 100 cubic feet of case files for final NMSS review.
.An NMSS contractor will. review about 1300 cubic feet of terminated Material License case files made available by IRM. The review will be based on an IRM forecast of the volume of decommissioning records contained in each' case file.

(4 Office of Consolidation Items of Interest Week Ending June 15,-1990 Second Building Status On June 4 GSA provided a counter-proposal to the develo>er's draft lease addendum and a proposed schedule for space planning and )uilding construction.
'We responded formally to GSA on June 11 after discussing various aspects of agreement or disagreement during the week. GSA did not schedule a meeting with WFH to complete the lease agreement drafting process but suggerced that a meeting.before Jaly 9 is unlikely.
The developer scheduled a meeting with the Montgomery County Planning Board-for July 12 to review the site plans for the second building and adjacent. lot.
We met with WFN, GSA, Dewberry & Davis, and various sub-contractors on June 7 and ' June 13 to review NRC requirements for TWFN that would affect base-building architectural and support systems design and features. Some of the issues under consideration which remain to be resolved in detail /are: scope and complexity of computer center design proposed by ID'l's consu', tant; specific requirements.for emergency generator (s) which determine size and
. number; uninterruptable power supply (UPS) system capabilities for computer center and operations center; energy management-systems for TWFN; and,
' exterior art proposal for the two-building complex.
OUFN Cafeteria Since the last report, the cafeteria area was painted, ceiling tile and lighting
-was-installed, kitchen and serting equipment was inste.11ed and partially tested, the lower portions of the exhavst duct were installed, electrical work, including tie-in to main building fire alarm system, was compir.ted and air-conditioning was activated.- The main exhaust fan for.the roof arrivej in the area and will be brought up.to the penthouse roof for'insta11ation next week._ The prime contractor is committed to finishing all construction and equipment installation for the cafeteria by July 1.
Handicap Ramps A sub-contractor failed to correct an error in the fabrication and installation of the handicap hand-rail to our satisfaction. The prime contractor has agreed to reject the product and re-do the job.

The purpose of,the RER was to review the effectiveness of the safeguards systems and determine if the Nine Mile security program, as implemented, met the performance objectives of 10'CFR Part 73. The review focused-particular attention on the methods and procedures em)1oyed to protect.

Callaway Nuclear Plant On June 12, 1990, Comissioner Kenneth Rogers toured the Callaway site with Mr. Edward G. Greenman, Director, Division of Reactor Projects, Region III, and members of the Region III staff.

-Palisades Nuclear Power Station-At'0555 on June 8,1990, Palisades experienced a failure of pressurizer heater transformer No. 15 caused by a short to ground. An Unusual Event was declared at 0615. Subsequent to discussions with the transformer vendor, the licensee determined that repair to the transformer at power
,was not practical and elected to shut down the plant and remain in hot-
-shutdown to determine a course of action. A spare transformer, which was to be used to provide additional power to support activities during-the steam generator replacement outage, will be temporarily used to repower the
. pressurizer heaters lost due to the failure of transformer No. 15.
In order'not to subject the plant to a thermal transient which is a potential contributor to steam generator tube degradation, the licensee has requested a Technical Specification Waiver of Compliance. The Waiver of Compliance-will extend the 72 hour8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> Technical Specification LC0 for an additional 7' day
-period to allow the plant to remain in hot' shutdown until the temporary transformer-can be installed. With the concurrence of ERR and the Deputy Regional Administrator, on June 11, the Director, Division of Reactor Projects, gave verbal authorization for th; gional Waiver of Compliance fat which time the licensee terminated th.
,asual Event. The verbal authorization was followed by a letter... firming the verbal authorization on June 13, 1990. The licensee expects to have the temporary transformer installed by Sunday, June 17th.

D. C.-Cook Nuclear Power Station 1
Unit 2 automatically tripped from 85 percent power at 1:17 p.m., on June-11, 1990.
The licensee investigation regarding.the root cause of theLtrip was not conclusive, however, an open phase on one of the power supplies for rod control in conjunction with the jogging of a power _ supply by a technician performing a fire protection surveillance is the most l probable cause. The licensee performed a very thorough investigation and reviewed the'results in a Plant Nuclear Safety Review Committee meeting
-prior to startup.
In addition to the trip, the licensee also identified a missing support for each RHR heat exchanger, which required. replacement prior'to plant restart. The supports were replaced and the licensee returned the plant to critical operations at 10:27 a.m., on June 14, 1990.

Cleanup of Tailings At Bluewater Mill, New Mexico On June 14, 1990, representatives of the Atlantic Richfield Oil Company (ARCO) met with the Director, Uraaium Recovery Field Office (URFO) and appropriate URF0 staff to discuss requirements and plans for cleanup of windblown tailings at the Bluewater Mill in New tiexico. The meeting was held at the URF0 office in Denver.

NPPD Management fleeting with RIV on Training Issues Nebraska Public Power District management met in the Region IV office on June 14, 1990, to discuss actions taken to address training program weaknesses.

HKil Associates Billings, f/.ontana The lice nee re)orted telephonically to the Region IV office that a portable gauge lad been run over by a roller at a construction site at Sheridt.n, Wyoming. The source rod became detached from the gauge. The t1RC staff uspects that the rod contains a 9 mil 11 curie sealed source of cesium-137 and that a 44 millicurie sealed source of americium-241 is housed within the case of the gauge. The licensee has cordoned off the site and has notified the gauge manufacturer who is sending a shipping container for return of the device. The Wyoming Bureau of Radiation l
Centrol has also been notified by the licensec. The licensee has been L
advised by NRC to handle the source rod remotely and to be wary of l
possible contamination. The licensee is attempting to obtain survey L
instrumentation from local sources.

Rancho Seco Nuclear Generating Station On June 13, 1990 SMUD General Manager David Freemt.n announced in a press conference the elimination of approximately 100 pesitions at Rancho Seco.
These reductions involve approximately 60 SMUD employees, 20 contract.
security personnel, and 20 other contract employees. These reductions will take effect through the summer. The major areas affected by the reductions in St1UD personnel include: Engiraering (25), QA (10), Radiation Protection (8) Training (7), Chemistry (5), Operations (5), and Licensing (4).
In the operations areas, 2 people holding reactor operating licenses were affected, but these were voluntary separations of the Plant Manager and an SRO.
There are only two reductions planned in the maintenance area involving mechanical, electrical, and I&C personnel.
