APPENDIX A Bergen-Paterson Pipesupport Company Docket No. 99900209/81-02 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on November 17-19, 1981, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:

Criterion V of Appendix 8 to 10 CFR Part 50 states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

A.

Paragraph NF-4231.2 of Subsection NF to Section III of the ASME Code states:

" Attachments which are welded onto the component support or parts during the process of manufacture or installation but which are not incorporated into the final support are permitted provided the requirements of (a) through (g) below are met.

(a) The material is identified and is suitable for welding but need not be certified material.

(b) The material is compatible for welding to the component material to which it is attached.

(c) The welding material is compatible with the base material and is certified in accordance with NF-2130.

(d) The welder and welding procedure are qualified in accordance with NF-4300.

(e) The immediate area around the temporary attachment is marked in a suitable manner so that after removal the area can be identified until after it has been examined in accordance with (g) below.

(f) The temporary attachment is completely removed in accordance with the procedures of NF-4211.

(g) The surface of the support is visually examined after removal of the temporary attachment."

Corporation Contrary to the above, the following was observed with respect to Subsection NF supports (Part No. 1001, Lot Code 028957cc, Purchase Order C38349):

(a) Temporary attachments of unidentified material were welded to the parts; (b) The qualification of the welder and welding procedure could not be established through available records; (c) The area around the temporary attachment had not been marked so that after removal the area could be identified; (d) No provisions had been made on the traveler to assure that the temporary attachments had been completely removed in accordance with the procedure of NF-4211; and (e) No provisions had been made on the traveler for the supports to be visually examined after removal of the temporary attachments.

Paragraph NCA-3820(b) of Section III of the ASME Code states in part, "A Material Supplier shall hold a Quality System Certificate (Materials) issued by the Society verifying the adequacy of the Material Supplier's Identification and Verification Program.

Alternatively, the Material Supplier shall have his Identification and Verification Program surveyed, qualified, and audited by the Certificate Holder for the items...."

Contrary to the above, purchase orders for ASTM A500, Grade B material were placed with a Material Supplier (Marmon-Keystone Corporation-Worchester, Massachusetts) who did not possess a ASME Quality System Certificate (Materials) and had not been surveyed, qualified, and audited by Bergen-Paterson.
