ENCLOSURE A - Paga 1 APPENDlX A NOTICE OF VIOLATION Power Authority of the State of New York Docket No. 50-333 As a result of the inspection conducted on November 1-30, 1981, and in accordance with the-Interim Enforcement Policy, 45 FR 66754 (Octob-r 7,.
1980), the following violation was identified:
10 CFR 50, Appendix B, Criterion ll, states in part, "The applicant shall Identify the structures, systems, and conponents to be covered by the quality assurance program... The quality assurance program shall provide control over activities -affecting the quality of Identified structures, systems, and components to an extent consistent with their importance to safety."
The quality Assurance Program, Section 2, paragraph 2.3, states, "The QA program shall apply to such activities as operation, testing, maintenance, mod i fi cat ion, repair and refueling of the plant. The program shall also apply to the safety-related structures, systems, and components defined in the Final Safety Analysis Report.
Quality Assurance Procedure.(QAP) 2.1, Revision 1, dated August 19, 1981, paragraph 5.4.1 states in part, "The quality Assurance program shall be applied as a minimum, to the structures, systems and/or components, delineated in Attachments 2.1-1 and 2.1-3 for JAFNPP and IP3NPP, respectively, whose failure could cause release of radioactivity exceeding the limits imposed by NRC regulations or whose function is vital to a safe shutdown of the plant and the removal of decay and sensible heat.".1-1 to QAP 2.1, James A. FitzPatrick Nuclear Power Plant, Safety-Related Structures, Systems, and Components lists the Reactor Building Crane and Refueling Platform and states (For system breakdown refer to equipment list).
Component data sheet 8-26 for the General Purpose Grapple indicates that it is QA Category I safety-related.
Contrary to the above, Work Request 08/15873, Refuel bridge fuel grapple proximity ' switch repai r performed on November 19, 1981 was classified as QA Category ll, non safety-related without any documented justification.
This is a Severity Level IV Violatior, (Supplement 1).

RESPONSE TO APPENDIX A - NOTICE OF VIOLATION 1)
Corrective action has been taken as follows:
a)
The System Component Indicles containing the QA Component Category have been reviewed and steps taken to assure that the classification assigned has not.been down graded for
-future application.
2)
Corrective action to prevent recurrence is being taken as follows:

a)
A controlled Component quality Assurance Category List is being prepared to prevent unauthorized changes.
b)
The Quality Assurance Procedure Manual is being' revised to identify and control the activities regarding a classification of components.
c)
A properly approved Plant Procedure will be issued to identify, control and provide measures for authorized changes to the Com-ponent Quality Assurance Category List.
3)
Corrective actica item I has been completed.
Corrective action items 2a, b and c shall be completed on or before April 1, 1982.
