As a result of the inspection conducted on July 22, 1981, and in accordance I [.- with the Interin Enforcement Policy, 45 FR 6C754 (October 7,1980), the i following violations were identified: y', A. License Condition 13.A. states that each sealed source containing -[4 .e> licensed material shall be tested for leakage and/or contamination -{ g at intervals not to exceed six months, f.- Contrary to this requirement, leak tests were not conducted at six 6 month intervals on a F0 millicurie strontium-90 eye applicator. Leak 'l. tests were not conducted during the following periods: July 1966 to February 3, 1967; August 1968 to February 20, 1969; March 1970 to January ?- 1972; February 1973 to January 1974; July 1974 to February 26, 1975; c $,g~ - and August 1975 to April 1977. This is a Severity Level V Violation (Supplement VII). ~ t ~' B. License Condition 15. states that the licensee shall possess and use licensed material in accordance with statements, representations,
