REPORTABLE OCCURRENCE 250-81-13 This memo confirms our telephone notification of the subject prcept notification reportable occurrence made Friday, July 10, 1981, to your Duty Officer, Mr. I. Y111alva as follows:.
OCCURENCE Eh Friday, July 10, 1981, a relatively small void discovered in the concrete issediately below the equipment hatch barrel was determined to be reportable in accordance with technical speciffcation 6.9.2.a.(9).
This void was found in the' course of makirg scoe alterations at the equipment hatch to facilitate c handling equipment and large components during the steam generator repair effort.
CORRECTIVE ACTION - Unit 3 Is presently defueled and undergoing
_ inservice inspection of the reactc vessel. Prior to restarting the unit an engineering evaluation D.urrently in progress) and any
- necessary repairs will be comple'.e;.
