This refers to the routine safety inspection conducted by Ms. V. P. Lougheed
[
of this office on October 30 through December 18, 1989, of activities at Fermi
Huclear Power Station, Unit 2, as authorized by HRC 0)erating License
No. NPF-43, and to the discussion of our findings wit 1 Mr. B. Sheffel and
other members of your staff at the conclusion of the inspection.

The purpose of this inspection was to review and witness activities dealing
with the performance of your first periodic containment integrated leak rate
test and local containment penetration leak rate testing. Within these areas,
the inspection consisted of a selective examination of procedures and
,
representative records, observations, and interviews with personnel as well
as independent computation of your leakage results..
The results of this inspection indicate that you have failed the periodic
integrated leak rate test due to excessive as-found penetration leakage.
Although the periodic test result is overwhelmingly dominated by the main steam
' isolation valve (MSIV) leakages, it should be noted that the test would have
failed even if the MSIVs had been removed from the as-found leakage penalty.
Since your as-left test results are below the 10 CFR 50 Appendix J requirement
and since Appendix J allows for an as-found failure, no additional actions are
'
necessary on your part in regard to the periodic test.

We consider the modification performed on the four in-board MSIVs to be a
positive ste) in an effort to reduce your local penetration leakage. However,
it appears t1at additional effort is necessary on your part with regard to the
other containment isolation valve leakages. This is based on the periodic
test results and the fact that the local penetration results (Type B & C
L
testing) have exceeded the appendix J requirements for the last two outages.
We will continue to monitor your results for the local penetration tests.
No violations of NRC requirements were identified during the course of this
inspection.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of
this letter and the enclosed inspection report will be placed in the NRC
Public Document Room.
