McGUIRE NUCLEAR STATION NOTICE OF VIOLATION 370/81-02-01 A.
10 CFR 50, Appendix B, Criterion XIII as bnplemented by DPC Topical Report Duke 1-A, Section 17, Paragraph 17.1.13 requires measures be established to control storage of equipment to prevent damage.
Contrary to the above, on February 3-6, 1981, adequate measures were not established to control storage af equipment to prevent damage in that:
1.
Four examples of rigging from insta.lled safety-related piping were noted.
2.
Three examples of rigging lines impinging on cable trays
.,c cables in trays were noted.
3.
Approximately eight examples of material stored in cable trays con-taining cables were noted.
4.
Five examples of scaffolding supported safety-related piping were noted.
Response
The violation occurred due to a lack of adequate guide lines for installing temporary scaffolding on permanently installed components and a failure to properly execute the proper procedure involving housekeeping inspections for storage of installed equipment.
A constrtttion procedure has been implemented which defines the guidelines and limitations to be used when placing temporary scaffolding on piping, hangers, equipment, and any other components permanently installed in the field.
The procedure specifically states that:
A.
No scaffold shall be supported from Schedule 10 pipe.
B.
No scaffold shall place any loads on valve operators, cable trays and cables, hangers such as snubbers, struts, and movable type hangers.
C.
No scaffold shall be supported by a pipe with O.D. of less than 3".
and in gensral, scaffold shall never damage any permanently installed item.
The procedure involving house keeping inspections for storage of installed equipment has been reemphasized and the appropriate personnel have been notified of the importance of this procedure.
The station is currently in full compliance.
