Enclosed is Certificate of Compliance No. 9039, Revision No. 4, for the Model No. 715 shipping package. This certificate supersedes, in its entirety, Certificate of Compliance No. 9039, Revision No. 3, aated December 1,1977.
Changes made to the enclosed certificate are indicated by vertical lines in the margin.
Those on attached list have been registered as users of this package under the general license provisions of 10 CFR 571.12(b) or 49 CFR il73.393a.
This approval constitutes authority to use this package for shipment of radio-active material and for the package to be shipped in accordance with the provisions of 49 CFR 3173.393a.
Please note that this approval has been renewed only until August 31, 1980.
In order to renew the approval beyond this date, we need a set of consolidated drawings for the package which shows only the significant safety features (e.g., dimensions,materialsofconstruction, welds, closures, seals, valves).
We will need sufficient copies of both th,e original consolidated application dated April 11, 1980 and the new consolidated drawings to make distribution to 4
all secondary users. There are currently fif ty four (54) registered secondary users. We will also require eight (8) copies of the consolidated drawings to accompany the consolidated application dated April 11, 1980 which you have previously submitted.
