APPENDIX.A NOTICE OF VIOLATION Long Island Lighting Company Docket No. 50-322 This refers to the inspection conducted by rerresentatives of the Region I (Philadelphia) Office at the Shorehan Nuclear Power Station, Shoreham, New York, of activities authorized by NRC License No. CPPR-95.
During this inspection, conducted on October 24-27, 1978, the following apparent items of noncompliance were identified.
I, 10 CFR 50, Appendix B, Criterion IX, states, in part, that:
" measures shall be established to assure that... welding (is)...
accomplished...in accordance with applicable codes, standards, specifications..."
The Shoreham FSAR, Table 9.1.4-2, paragraph 1.d, directs that the polar crane welds will be visually examined to the criteria of the AWS Code Dl.1 during shop fabrication.
SH1-99 Specification for Reactor Building Polar Crane states, in part, that: "The seller shall 100% visually examine all structural load bearing welds during the welding of the crane... the acceptance / rejection criter-ion for this visual examination shall be as specified in AWS Dl.1."
The AWS Structural Welding Code D1.1, 1974 edition, paragraph 3.6.4, states that: ".. undercut shall be not more than.. 1/32"..."
Contrary to the above, on October 25, 1978, general areas of under-cut in excess of 1/32" deep were observed on the trucks, bridge, and trolley welds of the reactor building polar crane.

10 CFR 50, Appendix B, Criterion V requires, in part, that: "Activi-ties affecting quality shall be prescribed by documented instructions, procedures,...,and shall be accomplished in accordance with these instructions, procedures..."
The Shoreham Nuclear Power Station FSAR Section 17.1.13D states, "the requirements for handling, storage, and shipping which apply during the transition phase, comply to the requirements of Sections 17.1 A,17.1B,17.1C, and Section 17.2."
The Section 17.1.2A Quality Assurance Program states in part, "the Quality Assurance Program requires that activities which affect the quality of safety related structures, systems, and components be prescribed by written approved specifications, procedures..." Construction procedure CSI-13.1 part 5.1 requires that trash, litter and excess materials "... shall not be allowed to accumulate and create condi-tions that will adversely affect quality."

Contrary to the above, on October 19, 1978, the energized safety related 4160 volt switchgear 1R22*SWG103 in the 103 emergency switchgear room, elevation 25', control room building, did not comply with the requirements of procedure C.S.1 13.I.
There was an accumulation of dirt inside of the energized switchgear equipment which could create conditions that would adversely affect the quality of the component and the equipment operation.
