,e r, ~ SECTION 1 SYNOPSIS OF EVENTS At 15:21:00 on January 15, 1979, TMI-2 experienced a rapid decrease in condense. vacuum while at 20% thermal power. Due to the rapid loss of condenser vacuum the control rocn operator tripped the turbine at 15:21:21. Later, at 15:29:36, the reactor tripped on low pressure. Prior to the loss of condenser vacuun all operating parameters were normal. Initial system conditions are listed in Section 2. Earlicr at 14:21, 14:45 and 15:17, the turbina was trip tested to verify trip capabilities. Nothing unusual was observed during these turbine trips. Af ter the 15:17 turbine trip test the turbine was latched at 15:19. During this latch the throttle valves and the governor valves were closed. '.When the turbine control was put into AUTO, the governor valves began controlling and the throttle valves went ful; open. Normal operation:has the throttle valves controlling and tne governor valves full cpen. At this time (1;:21:00), it is theorized a steam pressure spike occurred that caused the Moisture Separator Relief Valve (s) to open. This coincides with a loud steam / water nixture flow noise heard by L.C. Rogers and G. Schaedel of R?/J.while in their trailer outside of the Unit 2 Complex. Immedia tely condenser vacuum drooped frca 28" to 21" while the condenser hotwell level went low. Operator action tripced the turbine at 15:21:21 when the vacuum dropped. Operator (sl were sent to the tacuum pumps to investigate. They observed that the vacuum pum" wece running but apparently had excess seal water since they were unusually quic At this time the operators began draining the seal water from the vacuun per-the vacuum pump discharge lines via the knock-out drums. The turbine trin ci. the intercept valves which decreased the amount of " leakage" from the condenser via the Moisture Separa tor Relief Valves. -This allowed the hot condenser vacuum to stabilize, while the cold condenser vacuum continued to decrease since the greater a T permitted the cold condenser to lag the hat condenser during the vacuum drop. The rapid vacuun loss allowed steam to enter the vacuum pump system with a much greater enthalpy th.n usual and the vacuum pumps began flooding. During this entire evolution the circ water system operated nornally. (See Section 7). At approximately 15:24:30 the atmosphere relief valves (MS-V3A/B) opened. At this time the condenser vacuum changed slope and continued decreasing at a slower rate. Condenser vacuum continued to decrease till 15:55 when the vacuum pumps were shutdown. Shortly af ter the atmospheric relief valves opened the steam generators level went low (10 !5:38) and the RC pressure increased (2240 psig) till at aporoximately 15:26 when 'he electrcmatic relief, RC-V-2, opened. The opening of RC-V-2 started reducing RC pressur e. At this time reactor power was being manually reduced frcm 20% to 8% RTP. Power had stabilized at 8% RTP for approximately two minutes when the reactor tripped on low pressure of 1900 psig. Reactor pressure did not stop decreasing until 15:31:30 (1770 psig). Concurrent with the atnospheric dump valves (MS-V3A&33) opening, the downstream expansion bellows MS-U7A&7B ruptured. This failure allowed ~ steam to enter the control buildirg area. The stean caused unit substation 2 44 and 2-34 to trip on ground fault. Forther details of the bellows failure and subsequent damage inspection, and repair are listed in Appendix A&3. ') ' f, L* m b
a* During the interval that condenser vacuum had decreased, a broken yoke on I turoine bypass valve MS-V26B kept the bypass valve from closing. This per-mitted stean to cuntinue entering the condenser which pressurized to 5-6 pounds at which tine the B Low Pressure turbine rupture disc ruptured. By approximately 1730, af ter emergancy repair to the rupture disc, vacuum pumps A and C were started and a vacuum of 28" attained at 1810. ~ SECTION 3 CONCLUSIONS 1. The throttle valves opened af ter the system was put in AUTO when they should have been controlling. 2. Atnospheric Relief bellows failed allowing steam to enter the control building area which caused danage to: a. Pressurizer Heater Control Panel #324 b. Unit Substations 2-34 & 2-44 Transformers. 3. MS-V263 (bypass valve) was found with a broken yoke. 4. One turbine rupture disc ruptured due to excessive pressure in condenser which occurred due to vacuun loss. 5. No Technical Specification limiting conditions for operation were violated during the transient. 6. Reactor tr'pped on low pressure, however, pressure was recovered prior to reaching the safety injection setpoint. /. SECTIONS 4 ACTIONS 1. Installation of new bellows system for atmospheric dump lines. This will be operationally tested prior to next criticality. 2. Repair pressurizer Heater Control Panel #324 prior to next heatup. 3. All safety related ccaponents in the steam affected area were checked for satisfactory operation. (See Appendix B) 4. Replace turbine rupture discs. 5. Replace MS-V263 (bypass valve) yoke prior to next heatup. 6. Both 2-34 and 2-44 unit substation transformers were sent off-site to vendor for repair. While the transformers were being repaired a temporary transformer was connected to power substation 2-34 and 2-44. Prior to heatup both normal transformers were installed and satisfactorily tested. Temporary transformers have been discon-nected. 7. Vacuco pumps were drained and returned to service within 2 hours l after condenser vacuum loss. 8.
