SALEM AND HOPE CREEK RESIDENT INSPECTION NOS. 50-272/92-13;
50-311192-13; 50-354/92-13
This letter transmits the findings of an inspection to assure public health and safety,
conducted by Mr. T. Johnson-Senior Resident Inspector and other members of the NRC
resident and regional staff at the Salem and Hope Creek Nuclear Generating Stations for the
period between September 13, 1992 and October 17, 1992. The findings of this inspection
were previously discussed with Messrs. M. Morroni and J. Hagan of your staff.
Based on the results of this inspection, it appears that one of your activities at Hope Creek
was not conducted in full compliance with NRC requirements, as set forth in the Notice of
Violation, enclosed herewith as Appendix A. You are required to respond to this letter, and
in preparing your response, you should follow the instructions in Appendix A. This violation
involved instances of personnel error, inattention to detail, and lack of questioning attitude.
Several recent similar examples, involving several functional areas, are detailed in Section
8.2.A of the enclosed report. These include two licensee identified non-cited violations.
Viewed collectively, these findings appear to indicate a decline in performance at Hope Creek
during the current refueling outage. Our staff discussed these matters with Mr. J. Hagan' and
others of your staff on October 29, 1992. In addition to responding to the Notice of
Violation, please provide your assessment of these performance deficiencies within 60 days of
the date of this letter.
In accordance with 10 CFR 2. 790 of the NRC's "Rules of Practice," a copy of this letter and
its enclosure will be placed in the NRC Public Document Room.
The responses directed by this letter and the enclosed Notice are not subject to the clearance
procedures of the Office of Management and Budget as required by the Paperwork Reduction
Act of 1980, Pub. L. No.96-511.
