A) failure to maintain procedures in accordance with Technical Specification 6.8.3, and B) a failure to adhere to established procedures while conducting plant~evolutions.

Pursuant to the provisions of 10 CFR 2.201, our response to the Notice of Violation is provided in Attachment 1.

  • ATTACHMENT l 10 CFR 2.201 INFORMATION PUBLIC SERVICE ELECTRIC AND GAS SALEM GENERATING STATION RESPONSE TO NOTICE OF VIOLATION Your [[letter::05000311/LER-1985-016, :on 850723,discovered That Boron Concentrations in Boric Acid Tanks & Boron Injection Tank Below Min Required Value.Caused by Leak in Check Valve Between Boric Acid Blender & Tanks.Concentrations Restored|letter dated August 22, 1985]] identified two Violations of the Technical Specifications.

As stated therein, Violation "A" was the result of a failure to maintain procedures in accordance with Technical Specifications in that an on-the-spot change to procedure CH-3.5.012, "Gas Decay Tanks -

Sampling" resulted in the use of an improper sample point for sampling the gas decay tanks prior to release.

Violation "B" resulted from a failure to follow procedures while performing plant evolutions, specifically,the transfer of feedwater control from manual to auto during plant startup.

PUBLIC SERVICE ELECTRIC AND GAS DOES NOT DISPUTE THE VIOLATION

THE ROOT CAUSE OF THE VIOLATION WAS A FAILURE TO PERFORM A VERIFICATION ANALYSIS PRIOR TO APPROVING REVISIONS TO THE GAS DECAY TANK SAMPLE PROCEDURES AND A SUBSEQUENT FAILURE TO PROPERLY EVALUATE THE COLLECTED ANALYTICAL DATA.

IMMEDIATE CORRECTIVE ACTIONS:

The on-the-spot change was deleted from the procedure on July 1, 1985.

A review of the plant vent monitor data for the period of April 12 through July 1, 1985 was conducted to verify that the releases made during this period presented no hazard to the public.

LONG TERM CORRECTIVE ACTION:

a)

That portion of the Chemistry Department Organizational Manual associated with procedural changes has been revised.

Future revisions to safety-related sampling, analysis or calibration procedures will require verification that the new methods produce results equivalent to, if not better than, the method being revised.

b)

A training program is being developed to provide radio-chemistry data evaluation skills to chemistry technicians and assistants.

This program will be presented to all chemistry technicians, assistants, and appropriate supervisory staff personnel by November 30, 1985.

c)

A discussion of procedural development problems was held with chemistry personnel, with the gas decay tank sampling procedure emphasized.

This was completed July 26, 1985.

d)

An additional discussion of this event, its implications, and actions required to prevent recurrence, was conducted as part of the chemistry departments continuing training in industry events.

This was completed on August 23, 1985.

ADDITIONAL INFORMATION Item B Public Service Electric and Gas is aware of the NRC's concern over the use of on-the-spot changes to procedures and has taken positive steps to improve the safety review process and to place stricter controls on the use of on-the-spot changes.

Appropriate changes to the Technical Specifications were submitted for NRC review, and have been approved and issued as amendments 62 and 33 to the Unit 1 and Unit 2 licenses, respectively.

Administrative Procedure (AP) 32, which controls the initiation of procedural changes, was revised and issued on September 3, 1985 to incorporate these new controls.

One of the key improvements is the requirement to use a Station Qualified Reviewer (SQR) for all procedure changes.

Workshop training sessions on AP-32 have been conducted for procedure change initiators, SQR's and department heads.

THE ROOT CAUSE OF THE VIOLATION WAS A FAILURE TO FOLLOW ESTABLISHED PROCEDURES

IMMEDIATE CORRECTIVE ACTION:

The individual involved was counselled and appropriate disciplinary action was taken.

4.

LONG TERM CORRECTIVE ACTION:

a)

An eight hour training session emphasizing control room communications, and command and control functions is being conducted for all licensed shift operators, including supervisory and non-supervisory personnel.

This training was completed on September 13, 1985 *

<.. v This training has been made a permanent part of the operator training/requalif ication progaram.

b)

The simulator training program has been upgraded to include the operation/interaction of the steam generator feedwater regulating valves and their associated bypass valves.

This modification was completed August 31, 1985.

c)

As with all personnel error related incidents, this item will be included in the appropriate operator training/requalification program.

Verbatim compliance with procedures will be emphasized.

This training has been scheduled, and will be completed by January 2, 1986.
