- COMBINED INSPECTION 50-272/80-27 AND 50-311/80-20 We have reviewed the report of the subject inspection conducted by Mr. P.S. Koltay of your office on October 14-17, 1980.
Our respons*e to the infractions identified in Appendix A are as follows:
Item A, Infraction Amendment 21 to License No. DPR-70 paragraph CS states that "the approved fire protection plan consists of *.* information submitted with letters of *** March 2, 1979."
The licensee's letter dated March 2, 1979 to the Nuclear Regu-latory Commission in response to request for additional infor-mation related to fire protection review No. 1 and 2 Units Salem Nuclear Generating Station, states that "We comply with the supplemental guidance contained in 'Nuclear Plant Fire Protection Functional Responsibilities Administration Controls and Quality Assurance,' dated June 14, 1977."
NRC doc"ument titled, Nuclear Plant Fire Protection Functional Responsibilities, Attachment No. 2, Fire *Brigade Training states:
"the training program should consist of *** c'.Lassroom instructions, pra:_ctice in* fire* fighting *.* use o;f emergency breathing equip-ment *..
Instructions should be provided to all fire brigade members *** "
Contrary to the above, the licensee's records indicated that approximately one-half of the listed fire brigade members have

12-1-80 not received training in fire fighting and in the use of emer-gency breathing equipment.
The,licensee' s records also indicated
- that on September 18 and 19,.1980 one out of the five persons assigned to the'fire brigade had no previous related training.
On 'october 14,.1980 one person assigned to the fire brigade was not qualified on emergency breathing equipment.
This item refers to Docket Nos. 50-272 and 50-311.

- 1.
New employees have been assigned to the fire brigade, as a result not all fire brigade members have received fire school ;or air breathing equipment training.
The shift supervision was not aware of the qualification status of the fire brigade members.
- 2.
All fire br:i,gade members have received fire hose and ex-tinguisher training and have been qualified in the use of air breathing equipment.
- Each shift now has the minimum required fire brigade members qualified.
- 3.
To prevent recurrence, the Shift Supervisor has been noti-fied that only qualified members will be assigned to the fire team.
A list of brigade member qualifications is maintained in the Shift Supervisor's Office.
Classroom instruction is an ongoing evolution which new employees will complete at the earliest possible time.;
The Operating Engineer will monitor training more closely.

We are in compliance now.
- Item: B,* 'In:fra;ction Technical Specification Section 3.7.ll states:
"All penetration fire barriers protecting safety related areas shall be func-tional... at all times*.
With *.* required penetration fire bar-riers nonfunctional, establish a continuous fire watch within one hour."
Contrary to the *above, fire door to the Unit 2 Mechanical Pene-trations Area and three fire doors connecting the 2A, 2B and 2C diesel generator control cabinet rooms were* found blocked open and no* continuous fire watch had been established.

The cause was due to station and contractor personnel becoming

12-1-80 lax to the requirements to maintain fire barriers in a safe position during the refueling outage.
- 2.
.To correct the item, an Operations Department staff member.
is designated to perform surveillance for fire barriers to identify deficiencies and ensure that the responsible de-partment performs the necessary corrective action.
This action will continue on a shift basis for the duration of the refueling outage.
- 3.
To prevent recurrence all station and contracted personnel have been directed, as addressed in the Station Manager's and Contractor Supervisor's letters, to comply with the. Fire Barrier. Requirements.
- 4.
We are in compliance now.
