27780 Blue Star Memorial Highway, Covert, Ml 49043 August 24, 1992 Nuclear Regulatory Commission Document Control D.esk Washington, DC 20555 GB Slade General Manager DOCKET 50-255 - LICENSE DPR PALISADES PLANT - FARRIS SAFETY RELIEF VALVE MODEL NUMBER 2745 PKD/S4 - 10CFR21 On July 21, 1992, the investigation of the premature lifting of RV-3128, Safety Injection Tank T-828 relief valve, determined the problem to be related to the manufacture of the valve.

The premature lifting of this relief valve on May 13, 1992, lowered the safety injection tank nitrogen overpressure below the required technical specification limit and placed the plant in a one hour technical specification action statement. Subsequent plant operation continued with heightened operator attention. A second premature lift occurred on July 1, 1992 after a plant trip.

Subsequent removal and testing of the valve determined the problem to be misalignment of the spring adjustment screw bore in the bonnet of the valve.

This misalignment created inconsistent relief valve setpoints and reset values.

The bonnet and spring were replaced and reproducible relief valve setpoints and reset values were obtained.

Palisades has not experienced prior problems of this nature, though we have had 23 similar valves installed and in service for more than 20 years.

The enclosed 10CFR21 report is being submitted in conformance with the requirements of 10CFR21.21(3)(ii). The same information was sent by facsimile to the NRC Operations Center on July 30, 1992.

The valve bonnet and sprin~ have been replaced on the subject valve.

No further generic correct1ve action is planned, see #8 below.

We believe this is an isolated occurrence.

We have queried the industry experience database and have not found any similarly reported problems.

This is the first occurrence that Palisades has had with this problem and it should be noted this valve operated in the plant for 20 years without prior problems.
