ENCLOSURE 1

NOTICE OF VIOLATION Tennessee Valley Authority Browns Ferry 1, 2, and 3 Docket Nos. 50-259, 50-260, and 50-296 License Nos. DPR-33, DPR-52, and DPR-68 During the Nuclear Regulatory Commission (NRC) inspection conducted on March 16 - March 27, 1992, a violation of NRC requirements was identified.

In accor-dance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1991), the violation is listed below:

10 CFR 50 Appendix B, Criterion III, Design Control, requires that measures shall be established for the identification and control of design interfaces and for coordination among participating design organizations.

These measures shall include the establishment of procedures among participating design organizations for the review, approval, release, distribution, and revision of documents involving design interfaces.

Site Procedure SSP-9.3, (formerly SDSP 8.11) Plant Modifications and Design Change Control, established the administrative controls and require-ments for design changes and plant modifications based on design changes.

Nuclear Engineering Implementing Procedure, BFEP Pl 89-06, requires that prior to return to service primary and critical drawings be revised and issued to incorporate the installed changes.

Contrary to the above, these requirements were not met for the following examples:

A construction contractor, without adequate authorization, signed for Browns Ferry Nucleai Engineering as design authority on design change notice W16836A, on twelve advanced authorized forms for field design change notice F16903, on temporary alteration control form 3-91-004-100, on drawing approval for work orders 91-44565-00 and 91-44565-01, and a 10 CFR 50.59 safety assessment for a temporary alteration.

During the troubleshooting of the 4160 volt loop after a power inter-ruption on January 13, 1992, it was discovered that a primary draw-ing had not been updated.

Several recent modifications had been installed and equipment energized for four months without updating t

the primary drawing.

This is a Severity Level IV Violation (Supplement I).

DPR-33, DPR-52, and DPR-68 Pursuant to the provisions of 10 CFR 2.201, the Tennessee Valley Authority is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC 20555, with a copy to the Regional Administrator, Region II, a copy to the NRC Resident inspector, within 30 days of the date of the letter transmitting this Notice of Violation. This reply should be clearly marked as a "Reply to a Notice of Violation" and should include for each violation:

(1) the reason for the violation, or if contest-ed, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date with full compliance will be achieved.

If an adequate reply is not received in the time specified in this Notice, an order or demand for information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken.

Where good cause is shown, consideration will be given to extending the response time.
