R. Baker, at 1005 hours0.0116 days <br />0.279 hours <br />0.00166 weeks <br />3.824025e-4 months <br /> on November 28, 1978.
At that time we reported that while investigating the cause of water coming from the upper containment spray header we identified two lower containment spray check valves which had been installed back-wards.
A followup report will be submitted by December 1:2, 1978.as-required by Technical Specifications Section 6.9.1.8.
