NOTICE OF VIOLATION Florida Power and Light Company St.
Lucie 1 and 2
Docket Nos.
50-335 and 50-389 License Nos.
DPR-67 and NPF-16 Ouring the Nuclear Regulatory Commission (NRC) inspection conducted on June 17 August 4,
- 1986, a violation of NRC requirements was identified.
The violation involved the failure to establish and implement procedures.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"
10 CFR Part 2, Appendix C ( 1986),
the violation is listed below:

- part, the following are typical safety-related activities that should be covered by written procedures; Administrative Procedures and Procedures for
- Startup, Operation and Shutdown of Safety-Related PWR Systems.
The follow-ing system is an example that should have an operating procedure; Feedwater System.
Contrary to the above, On June 25, 1986, operating procedure (Op) 2-070020, Rev. 8, Condensate and Feedwater System - Normal Operation, was not properly established or maintained in that the procedure had numerous errors - wrong valve
- numbers, incorrect valve descriptions, wrong valve positions, and Unit 1
referenced components on the Unit 2
procedure.
Additionally, the procedure was not implemented properly in that three valves which the procedure designated as "open" and that operators, when performing the procedure in April 1986, had initiated as "open" were found to be closed.

that a plant work order (PWO) was not issued for a deficiency on a
plant system (190 foot wind direction remote instrument) which had been identified in the plant "Out-of-Service" log on July 9.
This is a Severity Level IV violation (Supplement I).

DPR-67 and NPF-16 Pursuant to the provisions of 10 CFR 2.201, Florida Power and Light Company is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including (for each violation):
( 1) admission or denial of the violation, (2) the reasons for the violation if admitted, (3) the corrective steps which have been taken and the results
- achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending the response time.
