Mr. Donald B. Karner Executive Vice President Gentlemen:
The purpose of this letter is to confirm our understanding of the actions you intend to take,
- 1) at Units 1 and 2 regarding the Unit 3 reactor trip which occurred on March 3, 1989, and 2) at Unit 1 regarding the reactor trip which occurred on March 5, 1989.
This letter recognizes activities which have occurred at the site since the issuance of the March 7, 1989, Confirmatory Action Letter and thus supersedes that.letter.
Based on the discussions held between Mr. R.
Zimmerman, of my staff, and you, on March 28, 1989, it is our understanding that your actions wi-11 include, but will not necessarily be limited to, the following:
1.
You will incorporate the experiences learned from the March 3, 1989, Unusual Event at Palo Verde Unit 3 into your activities at Units 1 and 2
prior to restart of those units.
This will include confirmation that the atmospheric dump valves (ADVs), emergency lighting, and steam bypass control systems, are operable and that the reactor coolant pumps'ower supplies are reliably ensured.
2.
The causes of equipment failure during these events will be determined, to the extent practicable, and corrective action will be taken to prevent future occurrences, prior to restart of Units 1 or 2.
This evaluation will include a thorough review of the equipment history of these components.
3.
You will conduct a thorough investigation and obtain a full understanding of the Unit 1 March 5, 1989, reactor trip and define the pre-restart and post-restart corrective actions needed as a result of this event.

You will brief the NRC upon completion of your investigations and will not restart (enter Mode 2) the Palo Verde Unit 1 or Unit 2 facilities prior to receiving NRC concurrence.
If our understanding concerning your proposed actions, as summarized
- above, is not correct, you should promptly notify this office in writing.
