Harris of the St. Lucie Plant dated December 9, 1977, the licensee was instructed to assu.e th;t applications for the renewal of operator licenses contain statements indicating any area(s) where the applicant failed to participate in the accepted requalification program and indicate the actions taken or plar">ed to compensate for such nonparticipation.
Contrary to the above, applications for the renewal of two licenses were sub'itted on February 23, 1979, for operators who had missed one or mo;e required lectures, and no statement of the inadequacy nor of the corrective actions wie re included.
~Res onse Amended renewal applications for operator licenses were submitted detailing the deficiencies in the individual requalification program attendance for the personnel involved.
The Training Supervisor is aware of this deviation and will assure tha'u ure applications for operator license renewal will include inadequacies and deficiercies of the operator as defined in fir. Collins letter of 9/12/77.
