This refers to the special inspection conducted by W.*G. Rogers and M. S. Peck

on August 29 through October 29, 1991.

Th~ inspection included a review of

activities authorized for your Dresden facility. *At the conclusion of the

inspection, the findings were discussed with those members of your staff

. identified in the enclosed report~

Areas examined during the inspection are identified in the report.

.

Specifically, the circumstances surrounding the loss of secondary containment

integrity due to the failure of station personnel to properly operate the

reactor building trackway outer door seal were examined.

Based on the result of this inspection, two apparent violations were

identified and are being considered for escalated enforcement action in

accordance with the "Genera 1 Statement of Po 1 icy and Procedure for NRC .

.

Enforcement Actions" (Enforcement.Policy), 10 CFR Part 2, Appendix C (1991).

The failure to maintain a monitor in attendance while the reactor building *

. trackway inner door was open is considered an apparent violation of Te.chnical

Specification (TS) 6.2 and the failure to correctly translate the reactor

building trackway outer door seal modification into appropriate procedu~es and

instructions is considered an apparent violation of 10 CFR 50, Appendix B,

Criterion III. These apparent violations in conjunction with other recent

events that appear to have similar causes are being reviewed by NRC management.

Accordingly, no Notice of Violation is presently being issued for these inspection

findings.

In addition, please be advised that the number and characterization of

apparent violations. described in the enclosed inspection report may change as

a result of further NRC review.

You will be advised by separate correspondence

of the results of our deliberations on this matter.

No response regarding

these apparent violations is required at this time.
