INFORMATION REQUEST TO SUPPORT UPCOMING SAFETY CONSCIOUS WORK ENVIRONMENT FOLLOW-UP INSPECTION AT THE PALISADES NUCLEAR PLANT

Dear Mr. Vitale:

This letter is to request information to support our scheduled Safety Conscious Work Environment (SCWE) Follow-up Inspection beginning December 8, 2014, at your Palisades Nuclear Plant. This inspection will be performed in accordance with NRC baseline Inspection Procedure 93100 and will examine in more depth the SCWE-related issues that were identified during a Problem Identification and Resolution focused inspection that was documented in Inspection Report 2014009.

In order to minimize the impact that the inspection has on the site and to ensure a productive inspection, we have enclosed a list of documents required for the inspection.

The documents requested are copies of condition reports (CRs) and lists of information necessary to ensure the inspection team is adequately prepared for the inspection. The information requested prior to the inspection may be provided in either CD-ROM/DVD (preferred) or hard copy format and should be ready for NRC review by November 21, 2014.

The requested information can be provided to Mr. Alex Garmoe, the Lead Inspector.

This letter does not contain new or amended information collection requirements subject to the Paperwork Reduction Act of 1995 (44 U.S.C. 3501 et seq.). Existing information collection requirements were approved by the Office of Management and Budget, Control Number 3150-0011.

The NRC may not conduct or sponsor, and a person is not required to respond to, a request for information or an information collection requirement unless the requesting document displays a currently valid Office of Management and Budget control number.

Enclosure Requested Information to Support Safety Conscious Work Environment Follow-up Inspection Please provide all of the information on four individual compact discs, if possible.

Unless otherwise specified, the time frame for requested information is for the period of December 1, 2013, through the time the data request is answered. Please label any electronic files with file content information.

In addition, inspectors will require computer access to the corrective action program database while on site.

PROGRAM DOCUMENTS

1. Copies of current administrative procedures for the corrective action program, including procedures related to trending and the self-assessment program.
2. Copies of current administrative procedures for the employee concerns program (ECP).
3. Copies of any current administrative procedures related to the assessment of safety culture or SCWE.
4. Copies of the original and most recent revision of the SCWE action plan for the security organization.

ASSESSMENTS

5. Copies of all external and internal safety culture or SCWE assessments, audits, surveys, and investigations related to the security organization conducted since December 1, 2013.
6. Completed effectiveness reviews for any actions taken to address concerns with safety culture or SCWE in the security organization since December 1, 2013.

CORRECTIVE ACTION DOCUMENTS

7. A list of condition reports initiated related to concerns with safety culture, SCWE, or the work environment in the security organization since December 1, 2013.
8. A data table (or similar format) showing the number of CRs initiated in 2013 and 2014, sorted by department, including number of the CRs that were anonymous. Include the staffing levels for each department.

ADMIN

9. Copies of all slides and handouts used in meetings or presentations focused on SCWE or safety culture since December 1, 2013.
10. Copies of all communications provided to the security organization or site staff regarding SCWE or safety culture since December 1, 2013.
11. Meeting minutes from all Nuclear Safety Culture Monitoring Panel meetings since December 1, 2013.

Documents Requested to be Available On-site During the Inspection

12. A list of issues brought to the ECP and the actions taken for resolution.
13. A list of issues brought to the security ombudsmen and the actions taken for resolution.

Other

14. A copy of the latest security department organization chart.
