SUMMARY
OF FEBRUARY 20, 2013, MEETING REGARDING AN INSPECTION PROCEDURE 95002 INSPECTION On February 20, 2013, members of the Region III staff of the U.S. Nuclear Regulatory Commission (NRC) met at the Perry Nuclear Power Plant (Perry) with members of the licensees management team to discuss the licensees plans and current progress in addressing weaknesses identified in NRC Inspection Report 05000440/2012009. This report, issued on December 28, 2012, documented the inspection conducted according to supplemental Inspection Procedure (IP) 95002, Inspection for One Degraded Cornerstone or Any Three Inputs in a Strategic Performance Area. The inspection had been conducted to assess the licensees evaluation associated with a finding with low to moderate safety significance (i.e., a White finding) that was identified in the second quarter of 2011 after high radiation levels were inadvertently created during withdrawal of an irradiated nuclear instrument, the C source range monitor detector. The inspection also reviewed a White Performance Indicator (PI), the Occupational Exposure Control Effectiveness PI, which had turned White from Green after several problems occurred at Perry with control of access to high radiation areas. As discussed in a letter to the licensee from the NRC, dated March 4, 2013, a continuation of the supplemental IP 95002 inspection will be conducted in June 2013.
Two meetings were held between the NRC and the licensee. The first meeting was with licensee managers directly responsible for addressing the White inspection finding and the White PI and preparing for the 95002 inspection in June. Licensee representatives discussed, at a high level, the root cause evaluations and a common cause evaluation conducted in response to the original high radiation area control problems. Licensee representatives also discussed additional actions that had been taken or were planned to address the problems and to prepare for the 95002 inspection in June. The licensee director from the corporate staff who had recently been assigned to oversee the licensees efforts was introduced to the NRC representatives. The second meeting was with the Site Vice-President and the corporate director.
At the meetings, the NRC representatives discussed that the licensees preparations for the 95002 inspection could include the establishment of internal milestones, check-and-assess points for the licensee to gauge its progress and implement any needed changes, and any other indicators to measure performance and improvement progress. The NRC representatives also discussed that the members of the 95002 inspection team had been tentatively identified: the Region III Division of Reactor Projects Branch 5 Project Engineer, who would lead the team, a Region III Health Physics Inspector, a Region I Health Physics Inspector, a Senior Resident Inspector from another site, and a Resident Inspector from another site. The inspectors would be onsite for two weeks in June. The NRC and the licensee also tentatively agreed to hold the 2012 end-of-cycle public meeting on April 10, 2013. The NRC discussed that this meeting would be an opportunity for the licensee to present the status of its preparations for the June 95002 inspection.
