Additional space is provided at the end of this checklist for documenting other comments.
- 1. Does anchorage of equipment in the area appear to be free of potentially adverse seismic conditions (if visible without necessarily opening cabinets)?
- 2. Does anchorage of equipment in the area appear to be free of significant degraded conditions?
- 3. Based on a visual inspection from the floor, do the cable/conduit raceways and HVAC ducting appear to be free of potentially adverse seismic conditions (e.g., condition of supports is adequate and fill conditions of cable trays appear to be inside acceptable limits)?
Cable trays and piping in the area found to be rigidly supported.
- 4. Does it appear that the area is free of potentially adverse seismic spatial interactions with other equipment in the area (e.g., ceiling tiles and lighting)?

- 1. Does anchorage of equipment in the area appear to be free of potentially adverse seismic conditions (if visible without necessarily opening cabinets)?
- 2. Does anchorage of equipment in the area appear to be free of significant degraded conditions?
Several valves were identified with moderate surface corrosion: 2SWS-MOV160, 2SWS-MOVI62 and 2SWS-MOV152-1. CR-2012-14449, CR-2012-14450 and CR-2012-14452 were issued to address these conditions. Valve 2SWS-MO VI 61 is rather a heavy MOV that is attached to bonnet with 4-3/8" diam bolts. These bolts are moderately corroded. CR-2012-14455 issued to address this condition. Surface rust on valve 2BOC-AOV-IOOA I judged not to be a concern.

Cable trays identified to be rigidly supported
- 4. Does it appear that the area is free of potentially adverse seismic spatial interactions with other equipment in the area (e.g., ceiling tiles and lighting)?
Fire hose station presents potential interaction with Panel 2PU-47. The panel is not a CA T I panel and the potential interaction was judged acceptable.

SFGB Instructions for Completing Checklist This checklist may be used to document the results of the Area Walk-By near one or more SWEL items. The space below each of the following questions may be used to record the results of judgments and findings.
Cable trays in the area found to be rigidly supported.
- 4. Does it appear that the area is free of potentially adverse seismic spatial interactions with other equipment in the area (e.g., ceiling tiles and lighting)?

RCBX Instructions for Completing Checklist This checklist may be used to document the results of the Area Walk-By near one or more SWEL items. The space below each of the following questions may be used to record the results of judgments and findings.
Wall mounted supports for piping were identified to be in good condition.
- 3. Based on a visual inspection from the floor, do the cable/conduit raceways and HVAC ducting appear to be free of potentially adverse seismic conditions (e.g., condition of supports is adequate and fill conditions of cable trays appear to be inside acceptable limits)?
- 4. Does it appear that the area is free of potentially adverse seismic spatial interactions with other equipment in the area (e.g., ceiling tiles and lighting)?
