Enclosure Request for Additional Information Tennessee Valley Authority (TVA)

Request to Use Delta Air Supplied Suits

1. Based on a review of the attachments provided in the licensee's submittal, the manufacturer states (and the European Certification Authority assumes) the V4 MTH2 suit is authorized for single-use only. Please confirm that the suit will be discarded after one use.
2. The use of National Institute for Occupation Safety and Health (NIOSH) equipment requires users to notify both the vendor and other users of any identified defects. Since the V4 MTH2 suits are non-NIOSH approved equipment, the staff requests verification that TVA will take timely action to report any defects experienced with these suits to other U.S. users in the nuclear field. Please describe your plans for notifying other users of potential defects.
3. While the licensee committed to develop implementing procedures (and training lesson plans) for using the suits, please identify any of the manufacturer's technical information (i.e., Instructions for Use) and directions provided, that will not be required by procedures.

For each instruction identified, explain the rationale for not including it in the program's procedures/training (and any other compensating actions).
