Special Report - Safety/Relief Valve Position Indicators This special report is being submitted, pursuant to the requirements of Hope Creek Technical Specification (TS) Table 3.3.7.5-1, "Accident Monitoring Instrumentation" Action 80, due to having less than the required number of position indication channels for the "J" Safety/Relief Valve (SRV) Position Indicator for greater than 30 days.
On May 11, 2012, the "J" SRV tailpipe temperature indication failed upscale.
Troubleshooting activities determined that the temperature element failed open. The TS requires two SRV position indication channels per valve to be operable, consisting of one tailpipe temperature element and one acoustic monitoring probe., With one of the two required channels inoperable for more than 30 days, a special report is required to be submitted.
The failure has been determined to be inside the drywell and is not accessible during power operations. Repair activities are being planned for the first outage of sufficient duration.
