Title	Stuck Fixed Gauge Shutter
Event Description	The following information was provided by the Mississippi State Department of Health (MSDH) via email: The licensee, Tronox, LLC in Hamilton, Mississippi (Monroe County) reported a fixed gauge (Ohmart SHF-2-30 Serial Number 5319GK, 200 mCi Cs-137) [with a] stuck shutter to Mississippi State Department of Health on late Friday afternoon (9/23/22). It was discovered [on] 9/22/22 at 1557 CDT. The failure was identified during calibration activities being performed by a BBP representative who is an authorized Ohmart representative. The source shutter failure appears to be caused by age and corrosion. No exposure has been received by Tronox personnel and the unit did not leave its normal operating position. A certified Ohmart representative will replace the shutter in place. MSDH has asked for a timeline on the replacement, a current survey reading at the restricted area boundary, and what additional precautions have been taken to assure no unnecessary exposures.
