Docket No. 50-362 30-Day Report Licensee Event Report No.82-002 San Onofre Nuclear Generating Station, Unit 3 This submittal is in accordance with the reporting requirements of Section 6.9.1.13.b of Appendix A to Facility Operating License NPF-15.

It describes a reportable occurrence involving Limiting Condition for Operation (LCO) 3.9.4 associated with containment building penetrations.

A completed copy of LER 82-002 is enclosed.

On November 19,1982 at 0128, during initial fuel loading operations, water was observed to be leaking through the equipment hatch seal into Containment. As required by the Action Statement associated with LC0 3.9.4 and Emergency Operating Instruction S023-3-5.11, core alterations were immediately suspended.

Subsequent investigation revealed the existence of a gap around the perimeter of the hatch. The hatch had been secured by four bolts as required by LC0 3.9.4 but no specific guidance had been available to maintenance personnel for equipment hatch installation other than to put it in place and tighten the bolts.

The four bolts were re-tightened at 0243 on November 19, 1982, the hatch visually verified closed and fuel loading operations we:e resumed.

As corrective action to prevent recurrence, specific guidance for equipment hatch closure when preparing for core alterations will be developed and governing procedures will be revised by March 1,1983.
