CONDITIONS PRIOR TO OCCURRENdE:

Mode 1 - Rx Power 97% - Unit Load 1030 MWe DESCRIPTION OF OCCURRENCE:

On :ugust 27, 1981, an operator noticed that the insulation was off a duct outside the primary sample room.

The insulation formed a pen-etration barrier, so a continuous fire watch was' required to be established within one hour.

Action statement 3.7.11 was entered at 1440 hours0.0167 days <br />0.4 hours <br />0.00238 weeks <br />5.4792e-4 months <br />.

This occurrence constituted operation in a degraded mode in accordance with technical specification 6.9.1.9.b.

DESIGNATION OF APPAPONT CAUSE OF OCCURRENCE:

The insulation had been an obstruction to the door from the primary sample room due to improper installation.

ANALYSIS OF OCCURRENCE:

Technical specifi :ation 3.7.11 requires:

With one or more penetration fire barriers non-functional, establish a continuous fire watch on at least one side of the affected penetration within one hour.

CORRECTIVE ACTION

A continuous watch was established within one hour.

The insulation was properly reinstalled and at 1325 hours0.0153 days <br />0.368 hours <br />0.00219 weeks <br />5.041625e-4 months <br />, August 29, 1981, action state-ment 3.7.11 was terminated.
