Engineered Safety Feature Actuation System Containment Pressure Hi-Hi Channel 1 - Inoperable This report was initiated by Incident Report 81-270.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 95% - Unit Load 1030 MWe DESCRIPTION OF OCCURRENCE:

On July 22, 1981, the control operator noticed that channel 1 of the containment pressure Hi-Hi indicators was spiking.

The channel was declared inoperable and was bypassed within one hour.

A channel check was performed on the remaining c:tannels within one hour.

Limiting condition for operation 3.3.2, table 3.3-3, action 16 was entered at 1300 hours0.015 days <br />0.361 hours <br />0.00215 weeks <br />4.9465e-4 months <br /> on July 22, 1981.

This occurrence constituted operation in a degraded mode in accord-ance with technical specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

A steam leak from a nearby steam generator blowdown line was blowing on che transmitter for containment Hi-Hi channel 1.

ANALYSIS OF OCCURRENCE:

Limiting condition for operation 3.3.2, table 3.3-3, action 16 states that:

With the number of operable channels one less than the total number of channels, operation may proceed provided the inoperable channel is placed in the bypassed condition and the minimum channels operable requirement is demonstrated by channel check within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />.

The leak was covered and the channel stopped spiking.

The channel was declared operable and limiting condition for operation 3.3.2, table 3.3-3, action 16 was terminated at 0850 hours0.00984 days <br />0.236 hours <br />0.00141 weeks <br />3.23425e-4 months <br /> on July 23, 1981.

In order to isolate and repair the steam leak, it would be necessary to shutdown the plant and drain the steam generator.

Therefore, the steam j

leak will remain covered until the next refueling outage; at which time it will be repaired.

No supplemental repcrt is necessary.
