OCCURRENCE:

On December 21.and 22, 1982, with the reactor shutdown, seven releases were made from the Reactor Building sump in the automatic mode.

During these

releases, the proportional sampler was inoperable, and daily samples were not taken.

This constitutes a degraded mode of LCO 4.8.3(f) and is reportable per Fort St. Vra.in Technical Specification AC 7.5.2(b)3.

EVENT

DESCRIPTION

With the reactor shut down and the Reactor Building tump proporticnal sampler inoperable, seven separate automatic releases were made from the Reactor Building sump between the hours of 0600 on December 21 and 0800 on December 22, 1982.

During' times that the continuous proportional sampler is inoperable and the Reactor Building sump ptaps are in the automatic mode, daily samples are required to be taken from the sump and analysis made of the composite sample three times per week. On December 21 and 22, the daily samples were inadvertently not taken.

As required, however, each of the seven discharges were made at a i

release rate less than 10 GPM, monitored by two activity monitors, recorded, and diluted with a minimum fresh water flow of at least 1100 GPM to assure conformance with the limits of 10CFR20.

No activity was detected during any of the discharges.

CAUSE

DESCRIPTION:

Due to an oversight by on-shift Radiation Protection personnel, samples of Reactor Building sump effluent were not taken on a daily basis as required when the proportional sampler is inoperable and the Reactor Building sump pumps are in the automatic mode.

Daily sample taking resumed on December 23, 1982.

The individuals responsible for obtaining the December 21 and December 22 samples were reminded of their responsibility by the Radiation Protectior Manager.

All Radiation Protection personnel responsible for taking Reactor Building sump samples received retraining via the Health Physics Order Book as to the requirements of Specification LCO 4.8.3, Reactor Building Sump Effluent, including the requirement to obtain samples on a daily basis.

Change Notice 1433 has been issued to the Nuclear Engineering Division. This Change Notice, inter alia, will replace the existing proportional sampler with a more reliable proportional sampler.

Incorporation of the new sampler into the system will reduce the probability that daily sampling will be required.

No further corrective action is anticipated or required.
