Inspection Scope The inspectors performed a review of the licensee's Corrective Action Program (CAP) and associated documents to identify trends that could indicate the existence of a more significant safety issue. The inspectors' review was focused on repetitive equipment issues, but also considered the results of daily inspector CAP item screening discussed in Section 4OA2.2 above, licensee trending efforts, and licensee human performance results. Of particular focus were equipment and performance issues where procedure use and adherence were a cause or a contributing causal factor. The inspectors' review nominally considered the six-month period of June 1, 2009, through November 30, 2009, although some examples expanded beyond those dates where the scope of the trend

warranted. The review also included issues documented outside the normal CAP in repetitive and/or rework maintenance lists, departmental pr oblem/challenges lists, quality assurance audit/surveillance reports, self-assessm ent reports, and management performance management tools. The review was performed as an independent review of the root cause evaluation contained in IR 950871, "Procedure Use and Adherence Issues Identified in Common Cause Assessment for 2009." The inspectors compared and contrasted their results with the results contained in the licensee's CAP trending reports.

Corrective actions associated with a sample of the issues identified in the licensee's trending reports were reviewed for adequacy. This review constituted a single semi-annual trend inspection sample as defined in Inspection Procedure 71152-05. b. Findings No findings of significance were identified.

Inclusion on this list does not imply that the NRC inspectors reviewed the documents in their entirety, but rather, that selected sections of portions of the documents were evaluated as part of the overall inspection effort. Inclusion of a document on this list does not imply NRC acceptance of the document or any part of it, unless this is stated in the body of the inspection report.
