intermediate
levelwastheerectionofanisolation
tentfortheremovalofasbestosinsulation
fromthepressurizer
shell.Priortothestart,ofthisjob,RG&Epipefitters
hadcompleted
repacking
RV-203onthepressurizer
intermediate
level.Airsamplesweretakenduringthevalveworkanduponcompletion
ofthejob,asmearsurveywasperformed.
Theresultsofthesurveyindicated
thatthecontamination
levelsweregenerally
high(40-80,000dpm/100cm),withtwosmearsdirectlyunderthevalveindicating
>100,000dpm/100cm.Sincetheadministrative
limitsforcontamination
applywhenthe"generalarea"contamination
levelsare>100,000dpm/100cm,thetechnician
didnotchangethepostingfrom"Contaminated
Area".Thiswasfurthersupported
bytheairsampleresultsfromthereliefvalveworkwhichwerewellbelowmaximumpermissible
concentrations
forairbornecontamination.
Thedocumented
surveyresultswerepassedoutofthecontainment
areasometimeduringthelatemorningorearlyafternoon
andplacedontheHealthPhysics(HP)Technician
Foreman's
desk.Theforeman,notingthetwosmearresults>100,000dpm/100cmdecidedtohavetheareapostingupgraded.
Thiswasaccomplished
byphoningoneoftheRG&Eleadtechsassignedtothesteamgenerator
inspection
andrepairjob.Atapproximately
1600hours,thecontaminated

theywereunnecessary.
Thetechnician
decidedtoallowthetwoworkerstousethemasks.(Thiswasinviolation
ofplantpolicy.)TechA,atapproximately
1740hours,turnedthecarpenter
crewcoverageovertoasecondContractor
Technician
(TechB).TechAverballypassedontheinformation
ondoses,contamination
levelsandindicated
thattwoworkerswouldbewearingmasksbyprefer-ence.TechBdidnotobtainandreadthecopyofthecontrolling
SWPutilizing
gusttheverbalturnoverforguidance.

1815hours,TechB,assumingthatintermittent
coveragehadbeenrequiredbytheworkpermit,providedaninitialsurvey,maskfittothetwoworkersrequesting
