oneitemwasfoundtobeinviolation.

wasidentified
asthefailuretocorrectcherootcausefortherepeatedpackingfailuresofaUnit2mainsteamtestselectorvalveoveraperiodoftwoyearsthatresultedinseveralentriesintofour-hour
limitingconditions
ofoperation.
Thisviolation
wassetforthinalettercontaining
thenoticeofviolation,
datedJuly20,1994,fromMr.E.G.Greenman,
Director,
DivisionofReactorProjects,
USNRC,RegionIII.TheletterwasreceivedJuly25,1994.Ourresponsetothenoticeofviolation
iscontained

tobeperformed
untiltherefueling
outagestartinginSeptember
1994.Consequently,
2-MMO-240
hadtoberepackedfivetimes,requiring
entriesintothefourhourLimitingCondition
ofOperation
(LCO),asrequiredbyTechnical
Specification
(TS)4.7.1.5.1.
Therewereseveralopportunities
sinceJuly1992toreplacethestemwithoutenteringtheLCO.ThisisaSeverityLevelIVviolation
(Supplement
I).

toreplacethestem.SincearepairactivityofthistyperequirescooldowntoMode5toperform,ascheduling
codeofU2R94wasassigned,
addingtherepairtothescopeofthe1994refueling
outage.AtthetimetheU2R94codewasassigned,
intheabsenceofpriorfailurehistory,thiswasthecorrectdesignation.
Thevalvestemreplacement
wouldonlyhavebeenelevatedtotheforcedoutagescheduleifthesubsequent,
repeatedrepairshadbeenidentified
asreworkandanadversetrend.Sincethevalvestemdeficiency
wasfirstidentified,
2-MMO-240
hasbeenrepackedfiveadditional
times.Hadthevalvestemreplacement
beenscheduled
asaforcedoutageactivity,
therewouldhavebeentwoopportunities
priortotheviolation
tocompletetherepair.Thefirstopportunity
occurredinAugust1993whentheunitexperienced
areactortripandforcedoutage.Thesecondopportunity
occurredinJanuary1994whentheunitwasremovedfromservicetorepairanunisolable
steamleakonSteamGenerator
21blowdownline.Repeatedpackingreplacements
werenotidentified
intheplant'sCorrective
ActionProgram(PMI-7030)
asanadversetrend.PMI-7030,

requiresacondition
reporttobewrittenforsimilarcorrective
maintenance
workwithin90daysoftheoriginaldeficiency.
OnlythefinalpackingfailureonMay17,1994,fellwithinthe90daycriteria,
