outage.AtthetimetheU2R94codewasassigned,

intheabsenceofpriorfailurehistory,thiswasthecorrectdesignation.

Thevalvestemreplacement

wouldonlyhavebeenelevatedtotheforcedoutagescheduleifthesubsequent,

repeatedrepairshadbeenidentified

asreworkandanadversetrend.Sincethevalvestemdeficiency

wasfirstidentified,

hasbeenrepackedfiveadditional

times.Hadthevalvestemreplacement

beenscheduled

asaforcedoutageactivity,

therewouldhavebeentwoopportunities

priortotheviolation

tocompletetherepair.Thefirstopportunity

occurredinAugust1993whentheunitexperienced

areactortripandforcedoutage.Thesecondopportunity

occurredinJanuary1994whentheunitwasremovedfromservicetorepairanunisolable
