ATTAQKENTTOAEP:NRC:1212IPage1BackgroundAroutinesafetyinspection,wasconductedbyMessrs.J.A.Isom,D.J.Hartland,D.L.Shepard,M.P.Huber,J.F.Schapker,andC.N.OrsinifromAugust13throughSeptember23,1994.Duringthisinspection,oneitemwasfoundtobeinviolation.Theviolationisassociatedwiththefailuretoremoveactionrequesttagsuponresolutionofthedeficienciesdocumentedonthetags.Thisviolationwassetforthinalettercontainingthenoticeofviolation,datedOctober21,1994,fromMr.E.G.Greenman,Director,DivisionofReactorProjects,USNRC,RegionIII.TheletterwasreceivedOctober27,1994.Ourresponsetothenoticeofviolationiscontainedwithinthisdocument.NRCViolation"TechnicalSpecification6.8.l.arequiresthatwrittenproceduresshallbeestablished,implemented,andmaintainedforapplicableproceduresrecommendedinAppendix"A"ofRegulatory.Guide1.33,Revision2,February1978.RegulatoryGuide1.33,paragraph9.e.lofAppendix"A,"requiresthatproceduresforthecontrolofmaintenanceincludeinformationsuchasthemethodforobtainingpermissionandclearancefor.operationpersonneltoworkandforloggingofsuchwork.ProcedureNPM-02CM,Revision4,"NuclearPlantMaintenanceSystemProcessInstruction,"requirestheloggingandremovingofactionrequesttagsassociatedwithequipmentrequiringcorrectivemaintenance.Specifically,paragraphs5.2.1.B.4and5.13.3.grequirethatallactionrequesttagsberemoveduponsuccessfulcompletionofthecorrectivemaintenanceoruponrejectionoftheactionrequestfollowinginitialreview.Contrarytotheabove,onSeptember26and28,1994,licenseepersonnelfailedtoremoveActionRequesttagsfromtheUnit2turbine-drivenauxiliaryfeedwater(TDAFW)pumptripandthrottlevalve(T&TV)andfromalocalcontrolpanellocatedintheUnit2"CD"emergency'ieselgenerator(EDG)despiteresolutionofthedeficienciesdocumentedonthesubjecttags.ThisisaSeverityLevelIVviolation(SupplementI)."
ATTACHMENTTOAEP'NRC:1212IPage2ResponsetoViolationForclarificationpurposes:ThenoticeofviolationindicatesthatanactionrequesttagwasnotremovedfromtheUnit2turbine-drivenauxiliaryfeedwater(TDAWP)pumptripandthrottlevalve(T&TV).Throughsubsequentdiscussionwiththeresidentinspector,ithasbeendeterminedthatitwastheUnit1TDAFW'pumpT&TVwhichwasinvolved,nottheUnit2TDAFVpumpT&TV.dmssioorDeniaotheAlleedVolatioIndianaMichiganPowerCompanyadmitstotheviolationascitedintheNRCNoticeofViolation,withnotetotheaboveclarification.2.easonsforteVplatoTheprocessatCookNuclearPlantfordocumentingandphysicallyaccountingfortheuse,location,andremovalofactionrequesttagsisnotadequatetoassuretagremovalafteractionrequestsarerejectedorworkhasbeencompleted.3.rectiveActosTaenadResultscievedTheactionrequesttagsthatwereidentifiedbytheinspectorasnothavingbeenremovedwereverifiedonOctober27,1994,tohavesincebeenremoved.4,CoectvectosakeovodteVioasTheplantprocesswillberevisedtoimplementadifferentmethodfordocumentingandphysicallyaccountingfortheuse,location,andremovalofactionrequesttags.ThisprocesschangeisscheduledforimplementationbyJune1,1995.Thisdateisbasedontheestimatedamountoftimerequiredtodevelop,review,andtrainonthenewprocess.Theconcernoffailingtoremoveactionrequesttagsisthatitmayinhibitpersonnelfrominitiatingnewactionrequestsormayresultinaninaccurateassessmentintheplantoftheconditionofplantcomponents.Historically,theplantactionrequesttaguse,location,andremovalprocesshasnotleadtoasignificantsafetyconcernorinaccurateassessmentofplantcomponents.Also,itisnotexpectedthatduringthetimetoimplementthenewprocessthatsuchconditionswouldevolve,basedonequipmentsurveillancerequirementsandtheabilityofplantpersonneltoaccesstheactionrequestsystemandevaluateactionrequeststatusindependentoftagplacement.
