Page2Contrarytotheabove,onMay3,1996,andNovember11,1997,theacceptancecriteriainmaintenanceprocedureM-15.1Mwerenotmetduringpost-maintenancetestingoftheB-emergencydieselgenerator(B-EDG).Inbothinstances,thedifferencebetweenthehighestandlowestfiringpressuresfortheB-EDGenginewasrecordedabovethemaximumallowablelimitspecifiedbyprocedureM-15.1M.Theunacceptabledatawasnotresolvedinaccordancewiththelicensee'sCorrectiveActionProgram,orjustifiedanddocumentedasacceptablebeforetheB-EDGwasreturnedto'ervice."(1),,thereasonfortheviolation,or,ifcontested,thebasisfordisputingtheviolationorseveritylevel:RochesterGas6ElectricCorporation(RGEE)acceptstheviolation.WeacknowledgethatEDGperformancedatawasnotproperlyresolvedrelativetoestablishedacceptancecriteriaandtheCorrectiveActionProgram.ThecollectionofEDGfiringpressuredataisbasedonguidancefromtheEDGvendormanual.Historically,suchdatawascollectedanddocumentedinMaintenanceProcedureM-15.1.2,"AorBDieselGenerator-ObtainCylinderFiringPressure'atFullLoad",att'esametimeasperiodictestingoftheEDGwasbeingperformed(usingSurveillanceTestProcedurePT-12.1orPT-12;2,"EmergencyDieselGeneratorA"and"EmergencyDieselGeneratorB")~ThedatacollectionwasscheduledpriortoplannedEDGmaintenanceoverhauls.ThisdatawasoneofthesoleindicatorsofEDGcondition,andwasprovidedtotheoriginalequipmentmanufacturer(OEM)fieldservicesrepresentativeforuseinplanninganyneededmaintenance.duringthesubsequentoverhaul.TheOEM'epresentative.iscontractedtoprovideguidanceandbe-presentduringtheEDGmaintenanceoverhaul.AftercompletionofEDGmaintenanceoverhauls,data(toverifythatproperadjustmentsandengineperformanceareacceptable)isobtainedduringtherestorationoftheEDGtoservice.ProceduresPT-12.1orPT-12.2areusedforpost-maintenancetesting(PMT)and,operabilityperformancetesting(OPT).DocumentationofthefiringpressureisrecordedinMaintenanceProcedureM-15.1M,,"AorBDieselGeneratorMechanicalInspectionandMaintenance",atthistime.Inthepast'woyears,GinnaStationhastakenadditionalmeasurestodetermineEDGengineconditionbycontractingwithanEDGengineanalysiscontractor.Thistypeofengineanalysishasgainedincreasingacceptancewithinthenuclearpowerindustry,andhasbeenendorsedbytheALCOOwner'sGroup.

Page3TheengineanalysiscontractorassistsRGSEpersonnelinperformingstate-of-the-artconditionmonitoring,bothpriortoandfollowingEDGmaintenanceoverhauls.Thismonitoringincludesenhanceddiagnostictesting,andprovidesmanyoftheparameterstoassistindeterminingengineperformance.Maintenanceprocedureshavenottotallyreflectedthenewmethodologyfordatacollectionanditsapplicabilitytoengineperformance.Thedatacollected,thatwasabovethemaximumallowablelimit,wasobtainedwiththeconcurrenceoftheOEMfieldservicesrepresentative,theengineanalysiscontractor,andtheSystemEngineer.~Thisdatawasreviewedinconjunctionwithdeterminingtheoverallengineperformance.Therepresentativewasknowledgeableoftheseparameters,andwasawareofthebasisforthefiringpressurelimits.Nevertheless,thisout-of-specificationdatawasrecordedandnotproperlyresolvedinaccordancewiththeGinnaStationadministrativeprocedures,inthattheRGEEmaintenancepersonnelinappropriatelyacceptedthedata,basedontheapparentacceptanceofthedatabythecontractorandengineeringpersonnel.(2)thecorrectivestepsthathavebeentakenand,theresultsachieved:0TheEDGmechanicwascounselledbytheManager,MechanicalMaintenanceonthepolicyforresolutionofdataoutsideacceptancecriteria.0ACTIONReport,97-2095wasinitiatedtoresolvethespecificdatawithintheworkpackageandtoinitiatecauseinvest3.gationfornotusingtheCorrectiveActionProgramforresolutionofthedata.0TheEDGswereretestedinFebruary1998inthepresenceoftheengineanalysiscontractor.RG&EandcontractorpersonneldeterminedtheoperationalperformanceoftheEDG'stobesatisfactory.(3)thecorrectivestepsthatwillbetakentoavoidfurtherviolations:oACTXONReport97-2095resultedinaTrainingWorkRequest(TWR),whichwasinitiatedtodevelopspecifictrainingintheproperresolutionofout-of-specificationresultsfromfielddatacollection.
