DuringanNRCinspectionconductedonFebruary8,1994toMarch8,1994,oneviolationofNRCrequirementswasidentified.Inaccordancewiththe"GeneralStatementofPolicyandProcedureforNRCEnforcementActions,"10CFRPart2,AppendixC,theviolationislistedbelow:10CFR50,AppendixB,CriterionXI,"TestControl,"states,inpart:nAtestprogramshallbe-establishedtoassurethatalltestingrequiredtodemonstratethatstructures,systems,andcomponentswill'performsatisfactorilyin.serviceisidentifiedandperformedinaccordancewithwrittentestprocedureswhichincorporatetherequirementsandacceptancelimitscontainedinapplicabledesigndocuments."Contrarytotheabove,twoconditionswereidentifiedthatdemonstratedthattestingwasnotbeingappropriatelyperformed.'pecifically:OnFebruary15,'1994,itwasdeterminedthatthetwomanualcontainmentisolationpushbuttonshadnotbeenperiodicallytestedeachrefuelingoutageasrequiredbyTechnicalSpecificationTable4.1.2item9.9406060037940525PDRADOCK05000244QPDR Letter:Subject:Date:Page2ViolationResponse94-05-01May6,1994OnMarch4,1994,thelicenseedeterminedthatcontainmentpressure(P)channelsP-947andP-948hadbeeninoperablesinceJune1992,duetoanobstructioninthecommoncontainmentpressuresensinglineforthepressuretransmitters(PT)-947andPT-948.Themonthlychannelfunctionaltestandannualchannelcalibration,asrequiredbyTechnicalSpecificationTable4.1-1,items17and25,associatedwiththesechannels,didnotdetecttheobstructedtubingduetotheobstructionbeinglocatedinanuntestedsectionofthepressuresensinglinebetweentheopenendinthecontainmentvesselandtherootisolationvalvesforPT-947andPT-948.Theresponsetotheviolationisprovidedseparatelyforeachofthetwoconditionsstated.A.ResponseforContainmentIsolationPushbuttons(1)thereasonfortheviolation,or,ifcontested,thebasisfordisputingtheviolation:RochesterGas&ElectricCorporation(RG&E)acceptstheviolation,andacknowledgesthatourtestprogramdidnottesttheentiremanualContainmentIsolationcircuitryeachrefuelingoutage,asrequiredbyTechnicalSpecificationTable.4.1-2,item9.RG&EformedaTechnicalSpecificationReviewTeamasaresultofLER93-005.(RefertoGinnaDocketNo.50-244,LER93-005.)ThisreviewteamwastaskedwiththeresponsibilityofidentifyingandreviewingprocedureswhichimplementTechnicalSpecificationsurveillancerequirementstoassureallSection4testrequirementswereproperlyimplemented.ThisreviewteamidentifiedadeficiencyinthattheContainmentIsolationpushbuttonsarenotfunctionallytestedeachrefuelingoutage.Uponself-identificationoftheomittedsurveillance,theuntestedContainmentIsolationpushbuttonwasimmediatelydeclaredinoperable,andtheappropriateLimitingConditionforOperation(LCO)wasentered.(2)thecorrectivestepsthathavebeentakenandtheresultsachieved:ProcedureRSSP-2.1,"SafetyInjectionFunctionalTest",hasbeenupgradedtoformallydocumentthetestingofbothmanualContainmentIsolationpushbuttons.ThistestwassuccessfullyconductedonApril7,1994,andwillbeperformedatthefrequencyrequiredbyTechnicalSpecifications.

lfLetter:Subject:Date:Page3ViolationResponse94-05-01May6,1994(3)thecorrectivestepsthatwillbetakentoavoidfurtherviolations:SincetheomittedsurveillancewasidentifiedasadirectresultoftheprogrammaticreviewoftheentireTechnicalSpecificationSection4surveillancerequirements,noadditionalcorrectivestepswillberequiredtoavoidfurtherviolations.(4)thedatewhenfullcompliancewillbeachieved:TheContainmentIsolationpushbuttonsweretestedonApril7,1994.Fullcompliancewasachievedatthecompletionofthistesting.B.ResponseforContainmentPressureChannelsthereasonfortheviolation,or,ifcontested,thebasisfordisputingtheviolation:RG&Eacceptstheviolation,andacknowledgesthatourtestprogramdidnotdetecttheobstructedtubing,whichwascausedbyrustaccumulation.Thiswasanisolatedcase,resultingfromwaterbeingretainedfollowingtransmittercalibration.Testingandcalibrationoftheassociatedvalvesandtransmittersisnormallyperformedtomeet10CFR50AppendixJ,ASME,andTechnicalSpecificationsrequirements.However,noneofthesetestsorcalibrationsweredesigned,todetectanobstructioninthepassivetubingfromthepenetrationtotherootvalve.Theobstructedtubingwasself-identifiedthroughaquestioningattitudeand.aggressivefollow-throughfromRG&Eemployees.Promptcorrectiveactionswereundertakenuponidentificationoftheproblem.ItshouldbenotedthatRG&Ewasintheprocessofrevisingthetestprocedureswhenthisconditionwasidentified.Theenhancedtestprocedurewouldhaveidentifiedtheobstructedtubing.Theenhancementwasunderwayaspartofourcontinuingefforttoimprovetestprocessesandprocedures.thecorrectivestepsthathavebeentakenandtheresultsachieved:ThemethodofcalibrationoftheContainmentpressuretransmittershasbeenchangedtousegas(insteadofwater)asthetestmedium.TheserevisedprocedureswereusedtoperformcalibrationsoftheContainmentpressurechannelsduringthe1994refuelingoutage.Continueduseofgasastestmediumwilleliminatethepotentialforobstructionscausedbyrustingofthepressuresensingtubing.
