EmergencyDieselGeneratorReportSouthernCaliforniaEdisonCompanySanOnofreNuclearGeneratingStationUnits2and3,DocketNos.50-361and50-362EventDate:September6,1990ThefollowinginformationisprovidedinaccordancewithTechnicalSpecificationSurveillanceRequirement4.8.1.1.3andRegulatoryPositionC.3.bofRegulatoryGuide(RG)1.108asrevisedbyGenericLetter84-15.RG1:108requestedthefollowingsevenitemsforeachvalidorinvalidtestfailure:1.ThisfailureinvolvedaninvalidtestoftheSanOnofreUnit2EmergencyDieselGenerator(EDG)2G003.2.Thiseventwasnotconsideredavalidtestof2G003.3.Duringapost-maintenanceoperabilitytest,EDG2G003trippedonhighcoolanttemperature.Thehighcoolanttemperaturewasaresultoftheradiatorfanpowersupplybreakersbeingopenedwithouteitherproperdocumentationorproceduraldirection.4TheradiatorfanbreakerswereclosedandEDG2G003retestedsatisfactorily.ArevisiontotheprocedurewillindicatethattheradiatorfanbreakersshouldbetaggedopenedwhentheDCcontrolpoweristaggedopened.Theresponsibleindividualswerecounseledontherequirementsformanipulatingsafetyrelatedequipment.AnOperatingDivisionExperienceReport(2-90-22)hasbeenpreparedinresponsetothisevent.5.Thisfailureoccurredduringaperiodofinoperabilitypreviouslyscheduledformaintenanceactivities.6.Thesurveillancetestintervalremainedat31days.7.ThesurveillancetestintervalwasinaccordancewiththescheduleofTechnicalSpecificationTable4.8-1.Page5of6

ray>~LEmergencyDiesel'eneratorReportSouthernCaliforniaEdisonCompanySanOnofreNuclearGeneratingStationUnits2and3,DocketNos.50-361and50-362EventDate:September21,1990ThefollowinginformationisprovidedinaccordancewithTechnicalSpecificationSurveillanceRequirement4.8.1.1.3andRegulatoryPositionC.3.bofRegulatoryGuide(RG)1.108asrevisedbyGenericLetter84-15.RG1.108requestedthefollowingsevenitemsforeachvalidorinvalidtestfailure:1.ThisfailureinvolvedaninvalidtestoftheSanOnofreUnit2EmergencyDieselGenerator(EDG)2G002.2.Thiseventwasnotconsideredavalidtestof2G002.3.Duringpost-maintenancefunctionaltesting,EDG2G002failedtodevelopeithervoltageorfrequencyandwasmanuallyshutdowninresponsetothe"breakeropen"and"undervoltage/underfrequency"alarms.Anothertestattemptwasmadeafterswitchingfromautomaticvoltageregulator(AVR)"B"toAVR"A".ThissecondtestresultedinanEDGtriponanover-excitationalarm.InvestigationofthefailurerevealedthatthelinepotfuseswerenotinstalledwhentheEDGwasbeingreturnedtoservice.Thisfailurewasadirectconsequenceofoperatorerror.4Ahumanperformanceeventevaluationwasinitiated(HPES90-021)andappropriatedisciplinaryactionwastaken.5.Thisfailureoccurredduringaperiodofinoperabilitypreviouslyscheduledformaintenanceactivities.6.Thesurveillancetestintervalremainedat31days.7.ThesurveillancetestintervalwasinaccordancewiththescheduleofTechnicalSpecificationTable4.8-1.Page6of6 CI+>>
