| Event Description | The following information was provided by the Maryland Department of the Environment via email:
On December 1, 2025, at about 1100 EST, the Maryland Department of the Environment's Radiological Health Program (MDE/RHP) was notified via phone and email from the radiation safety officer (RSO) of MD-07-014-01 (University of Maryland Baltimore) that 13.78 mCi of F-18 fluorodeoxyglucose (FDG) was used to a patient being evaluated for metastatic prostate cancer instead of F-18 prostate-specific membrane antigen (PSMA). The incident occurred on November 28, 2025, and the licensee is investigating the case and will provide a written report. MDE/RHP will finalize a reactive investigation. A Medical Event may indicate potential problems in a medical facility's use of radioactive materials. It does not necessarily result in harm to the patient. |
