'Upon replacement of the card reader he door was retested-and it was eterm d that a m still existed. The computer data base was checked and it was found that ad been programmed incorrect 1v.
Investigation revealed that this was one error when a computer technician utilized the wrong procedure when reprogramming a set of card readers for the Unit 2 Outage.

Written instructions have been implemented that detail the handling of defective keys to ensure compliance with item a) above.
4 A review of the meth4ds used to provide configuration control on the security system was conducted. The results of this review indicate that adecuate which provides control over these types of changes.
This method includes a review by Security Operations. Security I6C, Station Services Engineering and Computer Services. Final approval of modifications is done by the Security Supervisor.
