Dominion p, Energys Serial No.25-004 VCS LIC/JB/Rev 0 Docket No. 50-395 License No. NPF-12 INOPERABLE LOOSE-PART MONITORING SYSTEM CHANNEL Dominion Energy South Carolina, Inc. (DESC) hereby submits Special Report 2025-001, pursuant to the requirements of VCSNS Technical Specification (TS) 3.3.3.10 Action a (and TS 6.9.2). This special report is required because the VCSNS Loose-Part Monitoring System (LPMS) Channel 2 has been inoperable for greater than 30 days.
TS 3.3.3.10 requires the LPMS be operable in Modes 1 and 2. LPMS Channels 2 and 3 monitor the bottom of the reactor vessel. Channel 2 was declared inoperable on December 13, 2024, after failing its background check due to a degraded signal. LPMS Channel 3 continues to monitor this region of the reactor vessel for loose parts. Loss of dual-channel coincidence reduces one aspect of the false alarm filtering functionality.
The failed transducer for Channel 2 (IAT00752) is planned for repair or replacement during Refuel 29 in Spring 2026.
