INSPECTION GUIDANCE

  • ^

EF..

1.

Cocplete initial screening upon initial notification of all 12...

reportable events.

Initial notification may be by way of tele-

?"

phone, telegraph, facsimile, or written report.

EE-For purposes of this procedure, the generic term " event report" M.

is construed to encompass all reports of nonroutine or off normal events for which reporting requirenents exist.

These reporting requirenents are part of license conditions, Technical Specifications,

+=

or Title 10 Rules and Regulations.

Regulatory Guide 10.1 lists these reporting requirenents for all types of licensees.

}

The reporting requirements for events contained in Regulatory i!=

ifgE; ~

Guide 1.16 are a subset of all reporting requirenents in that they are associated only with Appendix A Technical Specifications

'.;g for reactor facilities, and they are the only requirenents for s

which the LER form specified in R.G.1.15 must be used.

MC--

Interpretations, R.G. 1.16 amplifies degraded' mode.

The expanded interpretation of event reports beyond the narrow scope of R.G.1.16 is used in this procedure to enable its use in reviewing E

all reports of events from all licensees, e.g., research reactors, fuel facilities, materials licensees, and reactors under construction.

Periodic, routine, or regularly recurring rep ~ orts of events, such as reactor building integrated leak rate test, startup of reactor,

"~

annuai operating report, etc., are not intended to be reviewed using this inspection procedure.

lb. If the event is considered to be a candidate for press release,

=

adv3.se the regional Public Af, fairs Officer that a press release

'.~

cay be warranted.

in If. MC 1110 delinates the criteria for determining whether an l

abnormal occurrence exists and establishes requisite IE Staff a

E= -

action upon identification of an abnormal occurrence.

2.

If the report is not cocplete, noi. understandable, or is considered E

to contain inaccurate information, contact the licensee and E

resolve the catter.

Obtain a connittent for a corrected report E=

if the information reported to the NRC is found to be significantly

[.

in error. If it is deternined that the incorrect information was r

purposely reported.or if the licensee frequently reports nisinfor-5" mation, enforcement action would be appropriate.

The threshold of

{g significance of errors including omission, above which a corrected report is required, involves inspector judgment, and should be f

commensurate with consideration of citing the licensee for failure

=. l to report, which infers failure to report accurately.

Errors of Mi 1 lesser significance s~ ould be discussed with the licensee with the

=

intent that future reports be correct, but insistence on submittal E

of a corrected report for the specific event is not warrante 1.

E-2b.

Of special significance is the cause cod'e on the LER form used

[

for cocputer entry.

If the cause of the event is determined to be incorrectly coded, the licensee should subtit a corrected LER

=-

form.

The LEI for= need be prepared only for eve.nts reportable g.t pursuant to R.G. 1.16.

2d.

~For purpose of gathering information regarding the extent of generic inpact of an event, telephone calls to other licensees I

within the region cay be beneficial. k'here deened appropriate by regional supervision, verbal co.=:unications should be utilized whenever there is a snbstantial yotential f.or generic applicability.

Kritten consunication to licensee-is not authorized.

Docu=entation of review of those events that were not followed up onsite should identify the event report and indicate or E-:.-

reference the action taken pursuant t'o the review. The M.[.

specifics of accounting and internal control of event reports under

' review is a regional prerogative.

The inspection report docu-EZ;',

mentation should distinguish between those events followed up j~

onsite and those reports closed out based on inoffice review g[M=

with no further' action necessary.
